ABOUT THE COMPANY
DHI 365 is a new and innovative system that provides dedicated, coordinated
care to the long\-term care community, specifically to patients residing in their
own homes.
We operate as an integrated healthcare model combining an independently
owned LTC pharmacy, telehealth physician services (not open to the public),
and remote patient care coordination. As a growing ecosystem, we are
expanding nationwide, currently operating in Pennsylvania and New Jersey.
Our focus is on improving patient outcomes, reducing risk, and supporting
individuals with chronic conditions through continuous monitoring, care
coordination, and proactive intervention.
JOB OVERVIEW
The Certified Medical Assistant (CMA) will be part of our clinical care
navigator team and play a key role in supporting ongoing patient care. They
will be responsible for day\-to\-day front and back\-office activities, with a
strong focus on patient communication, monitoring, and care coordination.
Such activities include regular correspondence between our physicians, our
patient population, and our care navigation team. Our CMAs provide remote
patient care by monitoring vitals, identifying potential concerns, and
maintaining consistent weekly communication with patients.
They assist physicians in conducting telehealth appointments, document
outcomes accurately, and help ensure continuity and effectiveness of care.
Additionally, they collaborate with Care Navigators to perform new patient
intakes, home assessments, and schedule device deployment and
installations. The CMA will also support the on\-call queue to provide timely
patient assistance when needed
RESPONSIBILITIES
- Perform consistent and professional patient outreach via phone
- Assist physicians in conducting telehealth appointments with patients,
- Monitor patient vitals and identify any changes or potential concerns that may
- Follow up with patients on the progress of their care on a weekly basis,
- Conduct outreach to patients in times of concern or when intervention
- Support Care Navigators with patient intake, onboarding, and home assessments
- Schedule device deployment and installations for patients based on diagnosis
- Follow all applicable government regulations, including HIPAA, and internal
- Other duties as assigned; job duties may vary depending on business needs
- High School diploma or equivalent required
- Certified Medical Assistant (preferred)
- Experience with both front office (patient interaction) and back office
- Experience providing patient care, including medication\-related support
- Familiarity with Electronic Health Record (EHR) systems (eClinical Works preferred)
- Strong attention to detail and accuracy, especially in documentation
- Excellent customer service and patient communication skills
- Ability to work effectively in a fast\-paced, structured, and team\-based environment
- Strong communication and interpersonal skills
- Problem\-solving mindset with the ability to recognize and escalate patient concerns
- Able to perform light physical lifting
- Reliable, consistent, and team\-oriented
- Valid Driver’s license and means of transportation
- Bilingual is a plus (but not required)
- Ability to sit for 6–8 hours per day
- Ability to lift and/or move up to 25 pounds
