ABOUT OUR COMPANY

Digital Health Information is a new and innovative system that provides

dedicated, coordinated care to the Long\-Term Care community, specifically

to patients residing in their own homes.

We operate as an integrated healthcare model combining an independently

owned LTC pharmacy, telehealth physician services (not open to the general

public), and remote patient care coordination. As a growing ecosystem, we

are expanding nationwide, currently operating in Pennsylvania and New

Jersey.

Our focus is on improving access to care, medication adherence, and

patient outcomes — especially for underserved and language\-specific

patient populations.

JOB OVERVIEW

We are seeking a Spanish\-Speaking Certified Medical Assistant (CMA) to

join our clinical care navigator team. The Certified Medical Assistant will be

responsible for day\-to\-day front and back\-office activities, with a strong

focus on supporting our Spanish\-speaking patient population.

Such activities include regular correspondence between our physicians,

Spanish\-speaking patients, and the care navigation team. Our CMAs provide

remote patient care by monitoring vitals, identifying potential concerns, and

maintaining consistent weekly communication with patients.

They assist physicians in conducting telehealth appointments, provide real\-

time translation for Spanish\-speaking patients, and document outcomes

accurately to support continuity of care and patient success.

Additionally, they collaborate with Care Navigators to perform new patient

intakes, home assessments, and schedule device deployment and

installations. The CMA will also support the on\-call queue to provide timely

and clear assistance to Spanish\-speaking patients when needed

RESPONSIBILITIES

  • Perform consistent and professional patient outreach via phone to the
  • Spanish\-speaking population

  • Assist physicians in conducting telehealth appointments and provide accurate,
  • real\-time translation for patients

  • Monitor patient vitals and identify any changes or potential concerns
  • that may require follow\-up

  • Follow up with Spanish\-speaking patients on the progress of their care
  • on a weekly basis, ensuring engagement and understanding

  • Conduct outreach to patients in times of concern or when additional support
  • may be needed

  • Support Care Navigators with Spanish\-speaking patient intakes
  • and home assessments

  • Schedule device deployment and installations for Spanish\-speaking
  • patients based on diagnosis and care plan

  • Follow all applicable government regulations, including HIPAA, and internal
  • care protocols

  • Other duties as assigned; job duties may vary depending on business needs
  • QUALIFICATIONS

  • Must be fluent in Spanish and English (spoken and written), with the ability
  • to clearly communicate medical and care\-related information

  • High School diploma or equivalent required
  • Certified Medical Assistant (2 years’ experience preferred)
  • Valid Driver’s license and means of transportation
  • Experience with both front office (patient interaction) and back office
  • (clinical support) activities

  • Experience providing patient care, including medication\-related support
  • and monitoring vitals

  • Familiarity with Electronic Health Record (EHR) systems
  • (eClinical Works preferred)

  • Strong attention to detail and accuracy, especially in documentation and
  • patient communication

  • Excellent customer service and patient communication skills, particularly
  • with diverse patient populations

  • Ability to work effectively in a fast\-paced, structured, and team\-based environment
  • Strong communication and interpersonal skills
  • Problem\-solving mindset with the ability to recognize and escalate
  • patient concerns

  • PHYSICAL DEMANDS

    The physical demands described here are representative of those

    that should be met by an employee to successfully perform the

    essential functions of this job:

  • Ability to sit for 6–8 hours per day
  • Ability to lift and/or move up to 25 pounds
  • Work environment consistent with standard clinical
  • and administrative operations

    PROPOSED SCHEDULE

    Mon–Fri, 9:00 AM – 5:00 PM (40 hours per week)

    COMPENSATION \& BENEFITS

  • $20\.00 – $24\.00 per hour (negotiable based on experience)
