Job Description
To partner with the clinical transplant coordinator team to organize pre and post transplant clinical and administrative support including, but not limited to, scheduling, verification of insurance, requesting medical records, and patient communications necessary to ensure a smooth, timely process for patients both pre and post transplant. Coordinates the escalation of patient issues and concerns to the transplant team.
Key Duties and Responsibilities
1\. Serves as a vital front\-line resource answering and triaging calls. Following established protocols, gathers basic information to assess need and determine urgency. Escalates call to clinical/specialist staff member as appropriate.
2\. Coordinates pre\-transplant evaluation functions. Initiates insurance verification for transplant evaluation and pre\-certification process for transplant related testing. Creates case, schedules evaluation testing, communicates with patient regarding scheduling and testing information, updates database with all relevant patient information collaborating with patient financial services to verify benefit coverage and secure patient account. Understands and interprets nuanced differences between in\-network and out of network transplant benefit coverage details to triage next steps in the transplant process. Gathers appropriate medical records from outside facilities. Generates patient chart.
3\. Coordinates post\-transplant activities. Completes lab requests prior to follow\-up appointments. Ensures that transplant database is updated with all lab values and that appropriate transplant coordinator is notified. Coordinates biopsy and infusion arrangements and communicates to clinical team members. Initiates pre\-certification process for transplant related testing. Communicates with patient regarding scheduling and
testing. Updates database with all relevant information. Gathers appropriate medical records. Generates patient chart.
4\. Coordinates with financial team to escalate transplant billing discrepancies.
5\. Attends QAPI meetings. Participates in quality improvement projects, relating EMR
documentation to quality metrics.
6\. Coordinates various administrative duties. Composes, types and proofs correspondence and inservice materials. Gathers data necessary to complete forms required by United Network of Organ Sharing (UNOS). Performs data entry for EMR and UNOS. Assembles
patient packets. Records minutes of patient selection meetings. Educates and enrolls patients in electronic patient portal.
7\. Performs other duties as assigned.
Qualifications
