Overview
We are seeking a Nurse Practitioner or Physician Assistant (Texas license required) to provide Independent Medical Opinions (IMOs), Nexus Letters, Medical Evaluations, and DBQs for veterans.
This is a remote, contract\-based role with flexible scheduling and minimal administrative burden. All cases include pre\-organized longitudinal medical history, structured templates, and drafted case framing, allowing providers to focus on clinical decision\-making and documentation.
Duties
- Conduct brief client phone calls ( 10–25 minutes)
- Review pre\-organized longitudinal medical history and records
- Complete Medical Opinions / Nexus Letters using a template, including:
- Clinical opinion
- Rationale
- Functional impact
- Etiology assessment
- Review of alternative explanations
- Perform Medical Evaluations, including establishing diagnosis when appropriate
- Complete DBQs (Disability Benefits Questionnaires), including:
- Diagnosis (confirm or establish)
- Functional impact documentation on standardized forms
- Utilize structured templates for all deliverables
- Ensure documentation is clear, clinically sound, and defensible
- Records review \+ brief client call
- Focused on service connection (direct or secondary)
- Most cases are standard; occasional complex cases may be compensated at a higher rate
- Records review \+ brief client call
- Typically includes establishing diagnosis
- Standardized form completion
- Covers diagnosis and functional impact
- Any 2 services → $300
- All 3 services → $400
- Conditions sharing the same causal chain are treated as one opinion
- Examples:
- Degenerative disc disease (DDD) with radiculopathy → one opinion
- Bilateral knee conditions → one opinion
- Volume varies based on client demand
- Providers licensed in additional states may receive increased opportunities
- Active Nurse Practitioner or Physician Assistant license in Texas
- Ability to conduct telehealth evaluations
- Strong clinical reasoning and documentation skills
- Experience reviewing medical records and forming defensible clinical opinions
- Licensed in multiple states
- Licensed in Alabama
- Experience with Independent Medical Opinions, Nexus Letters, or DBQs
- Experience with disability evaluations or medico\-legal documentation
- Strong ability to produce clear, well\-supported clinical rationale
- Licensed in clients state
- Malpractice insurance
- Ability to conduct telehealth using a HIPAA\-compliant platform (e.g. Google Meet with BAA, Zoom for Healthcare, or equivalent)
- Access to a secure, private environment for client calls and documentation

Compensation
Due to organization and preparation, typical clinician time on a case is less than one hour.
