ABOUT THIS OPPORTUNITY
Brookside Home Care is a growing private pay Licensed Home Care Services Agency (LHCSA) serving Nassau County, currently managing an active census of 30\+ patients. We are seeking an experienced Registered Nurse to serve as our Director of Patient Services — the sole clinical leader of our agency.
This is a meaningful leadership role for a nurse who thrives with autonomy, wants ownership over clinical operations, and is excited to grow alongside an expanding program. With NHTD and TBI program applications currently pending, the opportunity to shape something from the ground up is real.

WHAT YOU'LL BE RESPONSIBLE FOR
Clinical Leadership \& Compliance
\- Serve as the agency's Director of Patient Services per NYS DOH LHCSA regulations
\- Maintain survey readiness and lead all DOH compliance initiatives
\- Develop, review, and approve patient care plans and clinical documentation
\- Oversee quality assurance and performance improvement activities
Patient Care \& Field Visits
\- Conduct Start of Care (SOC) assessments for new patients
\- Perform recertification and RN supervisory visits
\- Coordinate care with patients, families, and physicians as needed
Staff Supervision
\- Supervise Home Health Aides (HHAs) and Personal Care Aides (PCAs)
\- Conduct required supervisory visits per NYS DOH standards
\- Lead aide orientations and in\-service training

IDEAL CANDIDATE
This role suits a nurse who is confident working independently and doesn't need a large team around them to be effective. You're organized, clinically sharp, and comfortable being the go\-to clinical voice for an agency you'll help build.
\- Experienced in NYS LHCSA compliance and DOH regulations
\- Comfortable as the sole clinical operator in a small agency
\- Strong supervisor of HHAs and PCAs
\- Proactive with documentation and deadlines
\- Flexible availability including occasional evenings or weekends
\- Interested in growing with the agency long term

QUALIFICATIONS
\- Active NYS Registered Nurse (RN) license in good standing
\- Minimum 3 years home care experience; LHCSA background strongly preferred
\- Demonstrated knowledge of NYS DOH regulations and survey process
\- Experience supervising HHAs and/or PCAs
\- Strong organizational skills and consistent follow\-through on deadlines
\- Ability to work independently with minimal administrative support

SCHEDULE \& STRUCTURE
\- Part\-time hybrid role
\- Combination of administrative oversight and field visits
\- 1 day per week in\-office (Lynbrook) for team coordination and orientations
\- Flexible scheduling based on patient census and clinical needs

COMPENSATION \& BENEFITS
\- $85,000 – $95,000 annually, commensurate with experience
