Job Summary
We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic home healthcare team. In this vital role, you will provide comprehensive assistance to patients who have returned home from a long term care at a nursing home.
Your expertise will help individuals navigate through weekly tasks around living independantly from scheduling doctors appointments to helping them pay their bills online, to scheduling transportation for their hospital visits or registering them for community programs or additional social services assistance.

Duties
- Implement crisis intervention strategies to support patients experiencing acute mental health emergencies or behavioral crises.
- Collaborate with medical professionals to coordinate integrated care plans while ensuring compliance with HIPAA regulations for medical documentation and patient privacy.
- Participate in case management activities including patient advocacy, resource linkage, home \& community care coordination, and follow\-up assessments.
- Experience in hospital settings or community\-based programs focusing on behavioral health or geriatrics; familiarity with Alzheimer's care is a plus.
- Knowledge of HIPAA regulations ensuring confidentiality and ethical handling of sensitive information.
- Capacity to work collaboratively within multidisciplinary teams while demonstrating cultural sensitivity and compassion for diverse populations.
