Now Hiring: Evaluation Coordinator Assistant
Join our growing team at E. Sholom, Inc. and play a vital role in supporting early intervention evaluation services. We are seeking a detail\-oriented and organized Evaluation Coordinator Assistant to help ensure timely, compliant, and high\-quality evaluation processes for children and families.
Position Overview
The Evaluation Coordinator Assistant works closely with the Evaluation Coordinator and interdisciplinary teams to monitor compliance with the 30\-day evaluation timeline. This role is essential in maintaining accurate tracking and documentation, facilitating communication across all evaluation team members and service coordinators, and ensuring all evaluation components are complete and submission\-ready.
Key Responsibilities
- Track MDE and Supplemental Evaluations for quality assurance and submit completed packets for review and entry into the BEI HUB portal.
- Conduct weekly follow\-ups with evaluators to ensure timely submission of reports and required consents
- Maintain detailed documentation of all communications with evaluators, families, and service coordinators in ProviderSoft (PS)
- Communicate evaluation status updates and resolve scheduling or documentation issues with Service Coordinators (SCs)
- Request and follow up on required medical forms and prescriptions from pediatricians for MDE evaluations
- Review evaluation reports for accuracy, completeness, and proper formatting (names, EI numbers, signatures, etc.)
- Ensure all evaluation consents (including telehealth/in\-person) are properly completed and signed
- Compile evaluation reports and consents into a single PDF for final review
- Distribute completed evaluation packets to families and upload documentation to PS
- Notify SCs upon BEI HUB submission and maintain tracking and submission logs
- Secure MDE Attestation Pages and confirm family communication has occurred
- Submit billing data sheets to the Billing Department upon completion
- Maintain organized electronic and physical records of evaluation packets
- Perform additional administrative and coordination tasks as assigned
- Strong attention to detail and organizational skills
- Excellent written and verbal communication abilities
- Ability to manage multiple deadlines and follow up proactively
- Experience with administrative coordination, healthcare, or early intervention services preferred
- Familiarity with documentation systems (e.g., ProviderSoft) is a plus
