Position Overview

We are seeking a Bilingual (English/Spanish) Medical Assistant for a remote Intake \& Care Coordinator role supporting patients in New York and Florida.

This is a hybrid role combining:

• Patient intake and registration
• Care coordination (CCM, RPM, TCM)
• Patient engagement and follow\-up

This role is ideal for candidates with experience in telehealth, or care management programs.

Responsibilities Patient Intake \& Registration

• Answer inbound and outbound patient calls (referrals, enrollments)
• Verify insurance (Medicare and Medicare Advantage)
• Complete patient registration and onboarding
• Schedule appointments in EMR (AthenaHealth preferred)
• Coordinate with physicians, home health agencies, and discharge planners

Care Coordination (CCM / RPM / TCM)

• Conduct monthly
Chronic Care Management (CCM) calls
• Support
Transitional Care Management (TCM) workflows (2\-day and 7\-day contact)
• Assist with
Remote Patient Monitoring (RPM) setup and patient education
• Document all encounters accurately for
CMS billing compliance
• Escalate clinical concerns to providers

Requirements

• Certified Medical Assistant (CMA/RMA) or relevant experience

* Fluent in English and Spanish (required)

• Experience with Medicare patients preferred
• Experience with CCM, RPM, or TCM strongly preferred
• Experience with AthenaHealth preferred
• Strong communication and phone skills
• Highly organized with strong attention to detail

Preferred Qualifications

• Telehealth or care coordination experience
• Experience working with elderly patient populations
• Understanding of CMS documentation requirements

Pay

$20 – $25 per hour (based on experience) and bonus tied to patient volume / retention.

Pay: $20\.00 \- $28\.00 per hour
