RAPID REHOUSING CASE MANAGER General Summary of Purpose of this Position:The Rapid Rehousing (RRH) Case Manager provides intensive, housing\-focused case management for individuals and families transitioning from homelessness into temporary or permanent apartments. This role guides clients through the entire housing stabilization pipeline, from initial intake screening and community diversion to long\-term self\-sufficiency. Operating within a trauma\-informed framework, the Case Manager identifies barriers to housing, creates goal\-oriented stability plans, and actively collaborates with landlords and community partners to prevent evictions. This position requires a high volume of field\-based work (home visits, inspections, and move\-ins) alongside rigorous documentation standards within HMIS. Specific Duties and Responsibilities: The statements describe essential functions but are not all\-inclusive. Individuals may perform other duties as assigned. Intensive Case Management \& Service Planning Conducts initial eligibility screenings, intake assessments, and diversion interventions to identify immediate alternative housing arrangements and personal resources. Maintains a consistent, active caseload of 15–20 individuals and families experiencing homelessness, extreme poverty, severe mental illness, or complex trauma. Partners with participants to develop, implement, and update individualized Housing Stability Plans tailored to their unique goals and self\-sufficiency timelines. Provides practical, person\-centered training in household management, credit repair, personal hygiene, medical adherence, and community integration. Conducts proactive, regular follow\-up home visits and monthly safety/wellness inspections to support clients, monitor progress, and reduce isolation. Exercises empathy, crisis intervention, and motivational support to assist clients resolving ambivalence regarding housing and stabilization opportunities. Housing Navigation, Landlord Relations, \& Advocacy Receives referrals through Coordinated Entry and orients participants on the legal rights, expectations, and responsibilities of tenancy. Assists participants with searching for, viewing, and securing permanent supportive housing or apartments of their choice. Collaborates directly with housing providers and landlords to compile necessary documentation, negotiate leases, and secure units. Serves as an ongoing liaison and mediator between landlords, property managers, neighbors, and participants to resolve disputes and proactively avoid evictions. Reviews and carefully explains residential lease contracts and housing regulations with households before signing. Transports and accompanies clients to critical housing\-related appointments, unit viewings, benefit hearings, and lease signings. Facilitates and physically assists with the client move\-in and move\-out processes, ensuring a seamless transition into the community. Compliance, File Management, \& Data Entry Input accurate, comprehensive, and timely client interaction data and services into HMIS and internal tracking tools in compliance with HUD standards. Secures and maintains live HMIS user credentials within 15 calendar days of hire. Generates and organizes complete, compliant physical and electronic client files, detailing all financial assistance, vouchers, and activity paperwork. Performs required annual or interim tenant re\-certifications, financial calculations, and property inspections to ensure proper vouchering for participant rents. Prepares and submits accurate monthly, quarterly, and annual program outcome reports. Upholds strict adherence to participant confidentiality protocols, agency policies, and OSHA safety standards. Working Schedule Standard Hours: Monday – Friday: 8:30 AM – 4:30 PM (or as determined by program operations). Schedule Disclosure: Programs operate to meet critical community needs. This position requires occasional schedule flexibility to work evenings or weekends for move\-ins, community events, or crisis responses. Physical Demands Ability to deliver household items and donations to clients, which requires lifting, carrying, and moving boxes of up to approximately 40 pounds. Capacity to perform physical field activities that require considerable use of arms and legs, such as walking, climbing stairs, balancing, stooping, and handling materials. Frequent local travel to multiple scattered\-site apartment complexes, shelters, and community agencies. Knowledge, Skills, and Abilities Deep understanding and operational commitment to the housing first philosophy, harm reduction strategies, and trauma\-informed care principles. Strong active listening, crisis de\-escalation, conflict resolution, and landlord\-tenant mediation skills. Excellent organizational abilities with a proven capacity to manage a demanding workload, complex record\-keeping, and strict compliance deadlines. Thorough knowledge of local community resources, social service networks, public benefits systems, and rental markets. Proficiency utilizing computers, including Microsoft Office Suite (Word, Excel, Outlook) and specialized database systems (HMIS / ClientTrack). Demonstrated dependability, initiative, and professional integrity when handling sensitive and highly confidential data. Valid driver’s license, reliable personal vehicle with adequate liability insurance, and an exemplary Motor Vehicle Record (MVR). Bilingual capabilities are highly preferred but not required. Qualifications Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related behavioral science field preferred. Experience: Minimum of 2 years of direct case management experience, ideally within the homeless services sector, rapid rehousing, or permanent supportive housing programs. Specialized Knowledge: Demonstrated experience working face\-to\-face with individuals navigating severe mental illness, substance use disorders, extreme poverty, or histories of trauma.
