Case Manager – DOL / EEOICPA Home CareLiberty HomeCare LLC – Oak Ridge, TN

Liberty Home Care LLC is seeking a compassionate, organized, and motivated Case Manager to join our growing team in Oak Ridge, Tennessee. We specialize in providing in\-home care services for Department of Labor (DOL) / EEOICPA patients and are looking for someone who is passionate about helping patients receive quality care while maintaining strong communication and compliance standards.

This position plays a key role in coordinating patient care, supporting caregivers, maintaining documentation, and ensuring excellent service for our clients and families.

Responsibilities

• Coordinate and oversee patient care services
• Communicate regularly with patients, caregivers, family members, and healthcare providers
• Assist with patient onboarding, scheduling, and care plan coordination
• Maintain accurate and timely documentation
• Ensure compliance with DOL / EEOICPA program requirements and company policies
• Monitor caregiver performance and client satisfaction
• Help resolve patient or staffing concerns professionally and efficiently
• Participate in care coordination and case review meetings
• Support office operations and administrative tasks as needed

Qualifications

• Previous case management, home health, healthcare coordination, or healthcare administration experience preferred
• RN, LPN, CNA, Medical Assistant, Social Work, or healthcare office experience is a plus
• Strong communication and organizational skills
• Ability to multitask and work independently
• Professional, dependable, and compassionate demeanor
• Basic computer and documentation skills required
• Experience with DOL / EEOICPA or home care is preferred but not required — willing to train the right candidate
