Job Description:
CARE Team is seeking a compassionate, organized, and dependable Care Coach to support family caregivers participating in Indiana’s Structured Family Caregiving (SFC) program.
In this role, you will provide ongoing guidance, monitoring, and support to caregivers who are caring for their loved ones at home. You’ll play a key part in ensuring caregivers feel confident, supported, and compliant with program requirements.
This is a part\-time, flexible position ideal for someone who enjoys building relationships and making a meaningful impact in the community.
Key Responsibilities:
- Conduct scheduled in\-home and/or virtual visits with caregivers and clients
- Provide coaching, education, and emotional support to family caregivers
- Monitor caregiver well\-being and client condition
- Ensure compliance with SFC program requirements
- Complete accurate and timely documentation in our electronic system
- Communicate with office staff regarding concerns, updates, or changes
- Participate in quarterly case management meetings as needed
- Experience in caregiving, home health, case management, or a related field preferred
- Strong communication and interpersonal skills
- Organized and detail\-oriented
- Comfortable using technology and documenting electronically
- Reliable transportation and valid driver’s license
- Ability to work independently and manage a flexible schedule
- CNA, HHA, Medical Assistant, LPN, Certified Health Coach, Case Management or similar background
- Experience working with Medicaid waiver programs
- Previous care coordination or coaching experience
- Part\-time (schedule your own hours) 10\-15 hours/week to start

* Hourly pay: $20–$24/hour, based on experience
