We are seeking a professional, organized, and detail\-oriented Administrative Assistant / Care Coordinator to join our team and support daily office and patient care coordination operations.

Position Summary

The Administrative Assistant / Care Coordinator plays a vital role in ensuring smooth office workflow, accurate patient intake, timely staff scheduling, and efficient communication with referral sources, clinicians, and healthcare partners.

This position is ideal for someone who thrives in a fast\-paced healthcare environment, has strong multitasking abilities, and enjoys coordinating both administrative and patient service operations.

Key Responsibilities

  • Admit new patients using WellSky software
  • Perform accurate data entry for incoming patient records
  • Monitor ongoing patient services and care coordination activities
  • Review clinical and service documentation submitted by healthcare staff in WellSky
  • Answer incoming calls from hospitals, rehabilitation centers, physicians’ offices, and referral partners
  • Schedule healthcare workers and clinicians for patient visits
  • Maintain organized filing systems and office records
  • Manage emails, online communications, and referral correspondence
  • Verify insurance benefits, authorizations, and patient eligibility
  • Maintain a clean, professional, and organized office environment
  • Qualifications

  • Proficient in Microsoft 365 applications
  • Strong working knowledge of Microsoft Outlook
  • Excellent verbal and written English communication skills
  • Strong decision\-making and problem\-solving abilities
  • Proven ability to multitask and prioritize effectively
  • Comfortable learning and using multiple software platforms quickly
  • Valid driver’s license with a good driving record
  • Minimum
  • 2 years of administrative office experience

  • Prior
  • healthcare administrative or home health experience preferred

  • Experience with WellSky is a strong plus
