Summary

The Community Health Worker (CHW) will work in collaboration with the University of Hawaii research team to implement a Produce Prescription (PRx) clinical trial aimed at addressing food insecurity and improving hypertension management among Waimanalo Health Center adult patients in the context of the Patient Centered Health Care Home (PCHCH). In the initial phase, the CHW will help plan and implement workshops in the community to identify ways to optimize the PRx program. Following this, the CHW will participate in training on hypertension management, nutrition and PRx program implementation. The CHW will support the program across all phases of the trial, from planning to implementation and conclusion of the trial.

This role involves a mix of administrative desk work (data entry, EMR monitoring) and direct patient interaction (phone calls, in\-person check\-ins). The role may require occasional evening or weekend hours to accommodate GMB workshops or participant schedules. Strict adherence to HIPAA regulations and human subjects research protections is required at all times.
