Summary:

The LEAD Mental Wellness counselor will provide support to the Program Director to implement, manage, and direct the operations and client services of the Mental Wellness Department.

The LEAD Mental Wellness will help as the Urban Jacksonville representative and aging/mental health resource among the community mental health providers as well as in the aging service community. This responsibility includes building community partnerships and collaborations, serving on community coalitions, and promoting the programs and services provided through the Mental Wellness Department.

The LEAD Mental Wellness counselor will also help develop new programs and funding streams that support the Mental Wellness Department's mission to "brighten the lives of older people by providing the most accessible and effective mental wellness interventions."

This position will also provide in\-home mental health assessment and counseling to address the symptoms arising from the stresses of the aging process and the functional limitations of that process. This service includes the development of appropriate personal support networks, exploration of possible alternative behavior patterns, therapeutic social skills, and identification of optimal interpersonal functioning. Counseling services include a treatment plan developed by the client and clinical social worker and in\-home counseling sessions to resolve the presenting problems.

Qualifications/Requirements:

  • Masters degree in social work, mental health counseling or marriage \& family therapy
  • Must have Licensed Clinical Social Worker (LCSW) license
  • Experience working with the geriatric population or in a mental health clinical setting
  • Strong written and oral communication skills
  • Computer skills with all Windows Applications helpful; word processing, spreadsheet accounting and database and power\-point programming experience.
  • Successful completion of all employment pre\-screening; background and DOEA checks, drug testing
  • Must have transportation and be able to travel throughout community
  • Valid Drivers license and auto insurance
  • Essential Functions:

  • Contact individuals or caregivers referred for assessment or counseling in order to obtain information regarding the individual's need for mental health services.
  • Obtain authorization to visit individuals in order to complete assessments.
  • Provide in\-home mental health assessments, mental health counseling and develop treatment plans.
  • Monitoring service delivery and providing crisis intervention.
  • Development of appropriate personal support networks, exploration of possible alternative behavior patterns, therapeutic social skills, and identification of optimal interpersonal functioning.
  • Will complete assessments, documentation, and billing for each service provided in a punctual manner.
  • Provide services to Medicaid recipients as requested.
  • Other duties as assigned by the supervisor.
  • Lead function:

  • Provide aging and mental health resource information and recommendations for community service providers and Urban Jacksonville staff.
  • Will train, support, and mentor interns and new staff in their goals of meeting or exceeding productivity expectations, providing best practice interventions, and meeting personal career development goals.
  • Will attend all training and personnel development activities required by supervisor.
  • Will maintain client confidentiality and privacy by strictly protecting all data and information provided by those served through Mental Wellness Department Programs.
  • Build community partnerships and collaborate with other community providers to ensure the unique needs of each client are addressed.
  • Serve as Urban Jacksonville representative on community coalitions.
  • Promote the Mental Wellness Department programs and services in the community and within Urban Jacksonville.
  • Will provide data, information, and technical assistance for monitors and funders.
  • Build community partnerships and collaborate with other community providers to ensure the unique needs of each client are addressed.
  • Serve as Urban Jacksonville representative on community coalitions.
  • Promote the Mental Wellness Department programs and services in the community and within Urban Jacksonville.
  • Will maintain client confidentiality and privacy by strictly protecting all data and information provided by those served through Mental Wellness Department Programs.
  • General Duties:

  • Will provide in\-home mental health assessment and counseling to older adults in the community who are referred for mental wellness services.
  • Specific Duties:

  • Will attempt to contact individuals or caregivers referred for assessment or counseling in order to obtain information regarding the individual's need for mental health services.
  • Will obtain authorization to visit individuals in order to complete assessment as necessary.
  • Will make home visits to individuals in order to complete mental health assessment as appropriate.
  • When assessment findings indicate that a client would likely benefit from counseling and the client is receptive to receiving this service, will provide counseling.
  • During initial counseling sessions, in collaboration with the client, will develop a treatment plan that establishes the client's goals for counseling.
  • May provide assessed individuals with referrals to appropriate services as indicated by assessment.
  • May coordinate with other agencies and professional individuals in obtaining services needed and desired by referred individuals.
  • Will collaborate with staff members from other UJI programs involved in the same cases as needed.
  • Will complete assessments, documentation, and billing for each service provided in a punctual manner.
  • Will maintain client confidentiality and privacy by strictly protecting all data and information provided by referred individuals.
  • Will attend all required training and personnel development activities required by supervisor.
  • May research, assemble, and summarize information from files and documentation in order to track outcomes and improve services provided through Mental Wellness Programs.
  • Working Conditions/Physical Requirements:

  • Business hours are Monday through Friday, 8:30 a.m.\-5:00 p.m.
  • Due to circumstances that may arise, overtime maybe required at times. In all cases, overtime must be pre\-approved by your supervisor.
  • Must be able to remain in a stationary position 50% of the time.
  • Requires sitting at a computer for up to eight hours a day.
  • The person in this position will occasionally (takes up to one\-third of their time) to move about inside the office to access file cabinets, office machinery, etc.
  • Constantly operates a computer other office productivity machinery such as a calculator, copy machine, and computer printer.
  • The person in this position frequently communicates with clients who have inquiries or questions. Must be able to exchange accurate information in these situations.
  • Frequently in client home settings.
