We are looking for a Part\-Time Home Visiting Patient Care Coordinator in the greater Houston area.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Make home visits to support virtual visits with provider
  • Make house visits as assigned by provider for unable to reach patients
  • Ensure the patients’ demographic information is correct and updated.
  • Assist patient with obtaining lab slips and/or referrals, scheduling procedures, prescriptions, etc.
  • Maintain a safe environment for patients and others.
  • Cooperate and interact with team members and co\-workers.
  • Educate patients on health care concerns and problems, office and hospital procedures, medications, testing, surgery, etc.
  • Request medical records.
  • Maintain patient confidentiality
  • Maintain patient charting, testing and flow sheets.
  • Organize and schedule appointments
  • EXPERIENCE

  • Willing to train a highly motivated individual
  • (Preferred)
  • Bilingual: (Preferred)
  • EDUCATION

  • High school Graduate or GED
  • Graduation from an accredited program for Medical Assistants (preferred)
  • CERTIFICATE/LICENSE

  • Certification from the American Association of Medical Assistants (preferred)
  • CPR certification
  • Job Type: Part\-time

    Pay: $18\.00 \- $25\.00 per hour

    Experience:

  • EMR systems: 1 year (Preferred)
  • Vital signs: 1 year (Preferred)
  • Language:

  • Spanish (Preferred)
  • License/Certification:

  • Certified Medical Assistant (Preferred)

Work Location: On the road
