Providence Medical Group (PMG) is seeking a Family Medicine nurse practitioner or physician associate to join an innovative Access Strategies team focused on expanding patient access and supporting clinic\-based primary care teams across Oregon.
This role is primarily virtual/remote, with most clinical work performed from home. Occasional in\-person presence may be required for team meetings, trainings, optional clinic days, or rare circumstances related to connectivity or operational needs.
Role Overview
In this innovative model, you will provide either:
- Direct patient care through scheduled virtual visits, or
- Indirect patient care by supporting physicians with in\-basket management and care coordination
- Full\-time (1\.0 FTE)
- Primarily virtual/remote role
- Shifts include extended hours with a rotating weekend schedule
- Virtual visits and/or indirect patient care
- Periodic team meetings or trainings in Portland
- Optional clinic\-based work if preferred
- Rare on\-site days related to connectivity or operational needs

- Licensed Nurse Practitioner or Physician Associate with primary care experience
- Minimum of 2 years of post\-graduate clinical experience in a primary care setting (required)
- 5\+ years of primary care experience preferred
- Reside within reasonable driving distance of the Portland Metro area
- Willingness to travel to Portland for occasional meetings or trainings
You will practice within a fully integrated, team\-based care model that includes behavioral health, clinical pharmacy, case management, and a highly experienced operational support team—allowing you to focus on quality patient care.
