Compensation:
Base Salary: $125,000 – $145,000
\+ Performance\-Based Bonuses
Total Earning Potential: $155,000 – $190,000\+ annually

Position Overview
We are seeking a licensed Nurse Practitioner (NP) in the state of Minnesota to join our growing remote care team. This role is ideal for a provider experienced in telehealth visits, chronic care management, and digital health platforms.
As a Nurse Practitioner, you will conduct virtual patient visits, support RPM program enrollment, and provide ongoing clinical oversight for patients managing chronic conditions.
Key Responsibilities
- Conduct telehealth visits (video/phone) with patients
- Perform initial patient establishment visits, including:
- Obtaining consent for RPM services
- Reviewing medical history and medications
- Develop and manage care plans for chronic conditions (e.g., hypertension, diabetes, CHF)
- Monitor and respond to RPM alerts and patient data trends
- Collaborate with:
- Pharmacists
- Supervising physicians
- RPM care team
- Document all encounters accurately in EHR systems
- Provide patient education on:
- Device usage
- Disease management
- Medication adherence
- Active, unrestricted Nurse Practitioner license in Minnesota
- National board certification (FNP, AGNP, or equivalent)
- Current DEA license (preferred but not required)
- Active NPI number
- Ability to work under a collaborative/supervising physician model (as required by state law)
- Minimum 1–2 years of clinical experience
- Experience with telehealth / virtual care delivery
- Familiarity with EHR platforms (e.g., CureMD or similar systems)
- Experience with Remote Patient Monitoring (RPM) or Chronic Care Management (CCM)
- Strong comfort with technology and remote patient engagement tools
- Strong clinical decision\-making skills
- Excellent communication and bedside manner (virtually)
- Ability to manage patient care independently in a remote setting
- Detail\-oriented with accurate documentation practices
