Position Summary:
Reporting to our Assistant VP of Medical Affairs, the Clinical Pharmacist will join our team on a full time basis to provides medication management, medication adherence, medication reconciliation and medication education services. Our Clinical Pharmacist positively impacts our patient's ability to manage their chronic condition. Minimum Qualifications:
- Doctor of Pharmacy Degree required from ACPE accredited college.
- NYS Licensed Pharmacist
- Postgraduate Year 1 (PGY1\) and/or Postgraduate Year 2 (PGY2\) in Ambulatory Care Residency preferred
- Bilingual Spanish/English highly desired.
- Takes Medical history of patient, medication reconciliation, and allergies.
- Provides patient education and counseling.
- Provides collaborative drug therapy management (CDTM) services.
- Maintains formulary, evaluates new formulary requests and maintains medication management policy and procedures.
- Provides tailor made therapeutic regimen, adjustment, and titration of doses for particular patients.
- Other clinical and administrative duties, as assigned.

Key Duties:
This position is a hybrid role, requiring onsite duties based in the Bronx, NY along with some remote/work\-from\-home capabilities. Compensation\- $120,000\-$125,000 contingent on experience based on 40 hours per week. Schedule: Monday to Friday 8:00 am to 5:00 pm. About Urban Health Plan:
