Mobile Wound Care Nurse Practitioner or Physician Assistant Field\-Based Advanced Practice Provider, Full\-Time, W\-2 Waltzman Plastic and Reconstructive Surgery, A Medical Practice, d/b/a BraveWounds. 4251 Long Beach Blvd, Suite 102, Long Beach, CA 90807\. Position Summary BraveWounds is a physician\-owned mobile wound\-care practice led by Joshua Waltzman, M.D., President and Chief Executive Officer and Medical Director. It is an early\-stage startup growing quickly, with more patients who need to be seen than clinicians to see them. This Advanced Practice Provider (APP) delivers advanced wound care in private homes, assisted living, board and care, skilled nursing, and partner facilities across Los Angeles and Orange Counties, with occasional visits in Riverside and San Bernardino Counties. The APP is the practice's lead field clinician: a daily visit route in a practice\-provided vehicle, notes complete within 24 hours, and virtual clinical direction of the practice's LVNs and RNs who deliver UltraMIST therapy, wound vac checks, and other delegated treatments. Role at a Glance Title: Wound Care Nurse Practitioner (NP) or Wound Care Physician Assistant (PA) Reports to: Dr. Waltzman (clinical); Practice Administrator (scheduling, payroll, vehicle, HR) Status: Full\-time, W\-2 employee, salaried, exempt (classification confirmed by counsel before start) Schedule: Monday through Friday, 9:00 a.m. to 5:30 p.m. Pacific (8 hours of work plus a 30\-minute meal break); occasional scheduled Saturday field visits; essential personnel, extra hours when patient care needs it Location: On the road across LA and Orange Counties (occasionally Riverside and San Bernardino), Dr. Waltzman's Long Beach office, and home when no visits or office work are scheduled Visit capacity: Up to 5 completed patient visits per day, scheduled and routed by the practice Base salary: $180,000 per year, paid semimonthly Annual bonus: Discretionary, up to 15% of base salary ($27,000\), on documented quality, documentation, and supervision criteria, at the sole discretion of the President and CEO Vehicle: Practice\-provided vehicle, fuel card, and toll transponder Benefits: 60/40 healthcare plan (practice pays 60% of the premium, employee pays 40%), two weeks PTO, California paid sick leave, CME allowance, professional liability coverage with tail Trial period: First 90 days. Four weeks' written notice from either side after the trial period Key Responsibilities Patient assessments Perform initial and follow\-up wound assessments: etiology, measurements, tissue type, exudate, periwound condition, and vascular screening with a portable Doppler. Order and interpret laboratory studies and imaging, diagnose within scope, and set the treatment plan and orders. Escalate red\-flag findings to Dr. Waltzman the same day. Conservative weekly visits Carry scheduled weekly wound\-care visits: debridement as indicated, dressing selection, negative pressure wound therapy management, compression and offloading, infection management, prescribing or furnishing within scope, and patient and caregiver education. Advanced modalities Deliver UltraMIST therapy and grafting with collagen\-based and amniotic skin substitutes: candidacy work\-up, physician review of each grafting case, application, product and lot documentation, and post\-graft follow\-up. Virtual LVN and RN supervision Serve as the directing clinician for practice LVNs and RNs delivering UltraMIST, wound vac checks, dressing changes, and other delegated treatments, using secure video, phone, and the EMR. Be reachable on nurse service days, hold a brief pre\-visit huddle, answer real\-time questions, decide when a visit must be converted to an APP or physician visit, review nurse notes within one business day, co\-sign where required, and take part in competency validations. Working on the road Travel is required and is part of the job. Drive a daily visit route in a practice\-provided vehicle to private homes, assisted living, board and care, skilled nursing, and partner facilities across Los Angeles and Orange Counties, with occasional visits in Riverside and San Bernardino Counties, completing up to five visits per day. Travel time between visits counts within the 8\-hour shift. Working at the practice office Work at Dr. Waltzman's Long Beach office for in\-office assessments and procedures, grafting cases scheduled with Dr. Waltzman, team meetings, chart review, training, and equipment and supply handling. Working from home When no visits are scheduled and no office work is necessary, work from home on documentation, LVN and RN supervision, care coordination, prior authorization support, and quality work, staying reachable and able to deploy the same day. Account clinical coordination Speak with the clinical personnel at the practice's accounts (facility nurses, Directors of Nursing, wound nurses, home health clinicians, caregivers) to coordinate care, issue and confirm treatment instructions and orders, and follow up on execution. Supply ordering Order your own supplies through the practice's corporate Amazon account and approved vendors, within the formulary and approval limits. The back office helps whenever needed. You never pay for practice supplies personally. Post\-visit supply debrief After each visit, one call or secure message to the back office listing the supplies, dressings, products, or equipment needed as soon as possible, so any gap is bridged before the next treatment. Documentation and Equipment Standards for Every Responsibility Complete and sign every encounter note within 24 hours in ChARM using the Heidi Health AI scribe, with patient consent recorded: history, measurements, labeled photographs, assessment, plan, orders, products applied with lot numbers, and education given. Complete physician\-directed amendments within 24 hours while preserving the original record and audit trail. Never fabricate, backdate, or make an unsupported amendment. Reconcile each note against orders, the prior visit, and demographics before signature. Carry the XSONX debridement device, portable Doppler, and the practice's field supply kit every field day, and the UltraMIST package when UltraMIST cases are assigned. Keep custody of equipment and biologics with logged handoffs and 24\-hour reporting of any discrepancy. Essential Personnel Expectations BraveWounds is an early\-stage startup in a high\-growth phase, and this is not a typical nine\-to\-five position. The scheduled day is 9:00 a.m. to 5:30 p.m., which covers 8 hours of work and a 30\-minute meal break, but the role is designated essential personnel and patient care does not stop at 5:30 p.m. or on Friday. The APP is expected to be available from time to time on Saturdays to see patients in the field when the practice schedules Saturday visits, to remain reachable by phone and secure messaging during core hours, to take reasonable after\-hours calls for urgent patient needs such as post\-graft complications and wound vac alarms, and to work additional hours, including occasional evenings or weekends, when patient care, LVN or RN escalations, facility requests, or audit deadlines require it. The practice will give as much notice as circumstances allow. The salary compensates all hours worked for an exempt employee. Qualifications Required Nurse Practitioner: active California RN license and NP certification, national board certification (AANP or ANCC), California furnishing number, and NPI. Physician Assistant: active California PA license, NCCPA certification, and NPI. At least two years of wound\-care experience, or comparable experience with debridement, skin substitutes, and negative pressure wound therapy. Valid California driver license, insurable driving record, and the ability to drive to up to five patient locations per day. Current BLS certification. Eligible to enroll with Medicare, Medi\-Cal, and commercial payers; not excluded from any federal or state health program. Comfortable working independently in the field, with strong clinical judgment and clear, timely written communication. Preferred Wound\-care certification (WCC, CWS, CWCN, or equivalent) or willingness to obtain it within 12 months. Experience directing LVNs, RNs, or other nursing staff, and experience in home health, skilled nursing, or assisted living settings. Familiarity with ChARM or a comparable EMR and with AI scribes such as Heidi Health. DEA registration, if the practice determines controlled\-substance authority is needed for the role. Tools, Systems, and Training The practice supplies the tool kit below and trains you on every item at onboarding and whenever a tool changes. The practice may add, replace, or retire any tool at any time; you are expected to adopt the designated tool by the date the practice sets. Heidi Health (AI scribe): Records the visit transcript with the patient's consent and drafts the clinical narrative. You review and approve the diagnoses and plan yourself before signing; billing codes are validated by the billing team, not the scribe. ChARM (EMR): Encounter notes, orders, labeled wound photographs, e\-prescribing, and amendments. A wound\-specialized EMR is under evaluation and may replace it. Secure messaging and telehealth: Virtual LVN and RN supervision, consults with Dr. Waltzman, and coordination with facility nursing. Practice email and phone: All patient, facility, and back\-office communication; personal email and texting are not used for patient information. Custody and vehicle logs: Equipment and biologics handoffs, lot tracking, mileage, and fuel card receipts. Corporate Amazon account and approved vendors: Ordering your own supplies within the formulary; the back office assists on request. Clinical kit: XSONX debridement device, portable Doppler, field supply kit, and the UltraMIST package when assigned. Working Conditions Daily driving in a practice\-provided vehicle; frequent entry into private homes and care facilities; standing, bending, kneeling, and lifting up to 30 pounds of equipment and supplies; exposure to bloodborne pathogens and medical waste under the practice's infection control protocols. Regular use of a laptop or tablet, secure phone, and telehealth tools from the field, the office, or a private home workspace. \-\-\- WHERE THIS ROLE CAN GO We are early, and we are growing fast. As the practice expands, we expect to create full\-time positions and clinical leadership roles, including paths such as Director of Nursing and Chief Nursing Officer. We are hiring now with those future seats in mind. We cannot promise a specific title or a specific date, and we will not pretend otherwise. What we can tell you honestly is that founding providers who do excellent work and help us build will be the first people we look to as those opportunities open up. \-\-\- LEGAL AND COMPLIANCE Independent Contractor Status This is a 1099 independent contractor engagement with BraveWounds, not employment. You control the manner and means of providing services, consistent with clinical standards and physician orders. You are not eligible for employee benefits such as paid time off, health insurance, workers compensation, or unemployment insurance. You are solely responsible for all applicable state and federal taxes. Non\-Compete and Confidentiality California law prohibits non\-compete agreements, and we do not impose any post\-contract restrictions on your ability to practice. We do ask that you maintain full confidentiality and HIPAA compliance throughout and after your engagement.
