Wound Care LVN, UltraMIST Specialist Field\-Based Licensed Vocational Nurse, Full\-Time, W\-2 Waltzman Plastic and Reconstructive Surgery, A Medical Practice, d/b/a BraveWounds. 4251 Long Beach Blvd, Suite 102, Long Beach, CA 90807\. \-\-\- Position Summary BraveWounds is a physician\-owned mobile wound\-care practice led by Joshua Waltzman, M.D., President and Chief Executive Officer and Medical Director. It is an early\-stage startup growing quickly, with more patients who need to be seen than clinicians to see them. This LVN is the practice's UltraMIST specialist: you deliver non\-contact low\-frequency ultrasound therapy at the bedside in private homes, assisted living, board and care, skilled nursing, and partner facilities across Los Angeles and Orange Counties, with occasional visits in Riverside and San Bernardino Counties. You work under physician orders with a Nurse Practitioner or Physician Assistant directing you virtually, you keep the UltraMIST system in service, and your notes are complete the same day. Role at a Glance Title: Wound Care Licensed Vocational Nurse, UltraMIST Specialist Reports to: Dr. Waltzman and the practice's directing Nurse Practitioner or Physician Assistant (clinical); Practice Administrator (scheduling, payroll, vehicle, HR) Status: Full\-time, W\-2 employee, salaried, scheduled 8\-hour days Schedule: Monday through Friday, 9:00 a.m. to 5:30 p.m. Pacific (8 hours of work, a 30\-minute meal break, and two paid 10\-minute rest breaks); occasional scheduled Saturday field visits; no overtime scheduled Location: On the road across LA and Orange Counties (occasionally Riverside and San Bernardino), Dr. Waltzman's Long Beach office, and home when no visits or office work are scheduled Daily route: Scheduled UltraMIST and wound\-care treatments, routed by the practice Base pay: $70,000 to $85,000 per year depending on experience, paid semimonthly Vehicle: Practice\-provided vehicle, fuel card, and toll transponder Benefits: 60/40 healthcare plan (practice pays 60% of the premium, employee pays 40%), two weeks PTO, California paid sick leave, continuing education allowance, professional liability coverage Trial period: First 90 days. Four weeks' written notice from either side after the trial period Key Responsibilities UltraMIST therapy delivery Deliver UltraMIST treatments exactly as ordered (site, duration, frequency) after competency validation. Set up, operate, clean, disinfect, charge, store, and transport the UltraMIST system and applicator heads per manufacturer instructions and practice SOP. Track applicator head use, keep cleaning and usage logs, and report any malfunction or warranty concern in writing within 24 hours. You are the steward of a device the practice depends on every day. Wound vac and dressing care Apply, monitor, and change negative pressure wound therapy (wound vac) dressings and perform other ordered dressing changes within LVN scope. Working under virtual direction Join a brief pre\-visit huddle with the directing NP or PA on service days, report findings in real time by secure video or phone, and route any conflict between an order and what you see to the ordering clinician before treatment proceeds. Escalate red flags the same day. Working on the road Travel is required and is part of the job. Drive a daily route in a practice\-provided vehicle to private homes, assisted living, board and care, skilled nursing, and partner facilities. Travel time between visits counts within the 8\-hour shift. Working at the practice office and from home Work at Dr. Waltzman's Long Beach office for scheduled treatments, team meetings, training, and equipment and supply handling. When no visits are scheduled, work from home on documentation and coordination, staying reachable and able to deploy the same day. Account clinical coordination Speak with facility nurses, Directors of Nursing, home health clinicians, and caregivers to confirm treatment schedules, pass along the clinician's instructions, and follow up on execution. Supply ordering and post\-visit debrief Order your own supplies through the practice's corporate Amazon account and approved vendors, within the formulary and approval limits; the back office helps whenever needed. After each visit, one call or secure message to the back office listing the supplies or equipment needed next. Documentation and Equipment Standards Complete every progress note the same day in ChARM per practice SOP: wound measurements, tissue description, periwound, exudate, pain, treatment parameters, patient tolerance and response, supplies used, and education provided. Capture and label wound photographs only in the approved workflow. Reconcile each note to physician orders and route any conflict or missing information to the ordering clinician before finalizing. Never fabricate, backdate, or make an unsupported amendment. Keep custody of the UltraMIST system, applicator heads, and field supply kit with logged handoffs and 24\-hour reporting of any discrepancy. Not part of this role: diagnosing, selecting or changing treatments or orders, sharp debridement or any procedure outside LVN scope, and billing or coverage determinations. Essential Personnel Expectations BraveWounds is an early\-stage startup in a high\-growth phase. The scheduled day is 9:00 a.m. to 5:30 p.m., which covers 8 hours of work, a 30\-minute unpaid meal break, and two paid 10\-minute rest breaks, with travel between visits inside the 8 hours. This is an 8\-hour job and the practice does not schedule overtime. The role is designated essential personnel, which means you are expected to be reachable by phone and secure messaging during core hours, to be available from time to time on Saturdays when the practice schedules Saturday visits, and to keep your route running when a facility or device issue comes up. Any work beyond the scheduled day requires the Practice Administrator's written approval in advance and is paid as California law requires. The practice will give as much notice as circumstances allow. Qualifications Required Active, unrestricted California Licensed Vocational Nurse license issued by the Board of Vocational Nursing and Psychiatric Technicians, and current BLS certification. At least one year of wound\-care experience, including negative pressure wound therapy (wound vac) management. Valid California driver license, insurable driving record, and the ability to drive to multiple patient locations per day. Not excluded from any federal or state health program. Comfortable working independently in the field, careful with equipment, and clear and timely in written communication. Preferred Hands\-on experience delivering UltraMIST (non\-contact low\-frequency ultrasound) therapy. If you have not operated the system, the practice trains you with the manufacturer's clinical specialist and validates your competency before you treat independently. Wound\-care certification (WCC or equivalent) or willingness to obtain it. Experience in home health, skilled nursing, or assisted living settings. Familiarity with ChARM or a comparable EMR. Tools, Systems, and Training The practice supplies the tool kit below and trains you on every item at onboarding and whenever a tool changes. The practice may add, replace, or retire any tool at any time. UltraMIST system and applicator heads: the primary treatment device in your custody on every field day. ChARM (EMR): progress notes, labeled wound photographs, and supply documentation. A wound\-specialized EMR is under evaluation and may replace it. Secure messaging and telehealth: virtual direction from the NP or PA, consults with Dr. Waltzman, and coordination with facility nursing. Practice email, phone, and tablet: all patient, facility, and back\-office communication; personal email, texting, and personal camera rolls are not used for patient information. Custody and vehicle logs: device and supply handoffs, cleaning and usage logs, mileage, and fuel card receipts. Corporate Amazon account and approved vendors: ordering your own supplies within the formulary. Field supply kit: dressings, wound vac supplies, and consumables for the day's route. Working Conditions Daily driving in a practice\-provided vehicle; frequent entry into private homes and care facilities; standing, bending, kneeling, and lifting up to 30 pounds of equipment and supplies; exposure to bloodborne pathogens and medical waste under the practice's infection control protocols. Regular use of a practice tablet, secure phone, and telehealth tools from the field, the office, or a private home workspace. \-\-\- LEGAL AND COMPLIANCE Independent Contractor Status This is a 1099 independent contractor engagement with BraveWounds, not employment. You control the manner and means of providing services, consistent with clinical standards and physician orders. You are not eligible for employee benefits such as paid time off, health insurance, workers compensation, or unemployment insurance. You are solely responsible for all applicable state and federal taxes.
