Department: --------------- CPH \| HBHE Lovelady Department's Website: Summary of Job Duties: -------------------------- The Community Health Coordinator (CHC) will provide peer support (status checks, encouragement/motivation, virtual and in\-person visits, outreach, and skill building activities) to current and recently discharged patients with a violent assault injury in the UAMS ED and Trauma Center as part of a local hospital\-based violence intervention research program, Project Heal. The CHC will assist various elements of the community violence prevention research program, including recruitment, data collection, management, and analytics (particularly both quantitative and qualitative methods), research design, grant writing, and dissemination (manuscripts, community newsletters, presentations). Primary duties will include ensuring brief bedside and peer support intervention components along with data tracking and other necessary research responsibilities. The CHC role requires effective communication with research participants, research and clinical staff, community partners, and national network members. Qualifications: ------------------- Bachelor’s degree in Health Education or related field or High school diploma/GED plus four (4\) years of health education experience required. Special Experience : This position is paid through a grant: UAMS Project Heal: A Hospital\-based Violence Intervention Program that requires the person hired to have previous experience of being violently assaulted (i.e., gunshot, stabbing, etc.) and treated in a hospital for penetrating injury and recovery to ensure the ability to build rapport and provide peer support to the program participants by drawing from lived experiences. Knowledge, Skills \& Abilities: At least 3 years of experience working with health care systems, victims’ services, mental health or substance use treatment programs or community\-based programs/organizations. Experience with engagement/customer service, recruitment, facilitating collaborative partnerships, client assessment, treatment or action planning, conducting conversational interviews/check\-ins, advising/counseling/mentoring/motivating clients, leading group discussions, and/or participating in health promotion and health education activities. Intimate understanding of and experience working with the community served by the individuals impacted by community violence. Ability to initiate and maintain positive working relationships with project staff and other organizations. Ability and willingness to provide emotional support, encouragement and motivation to program participants Ability to consistently uphold confidentiality and ethical practices when handling sensitive information. Ability to perform essential computer tasks and apply a working knowledge of common software applications, including Microsoft Office Suite (Word, Excel, Outlook, PowerPoint), to support daily operations and documentation needs. Ability to consistently report to work as scheduled and fulfill assigned hours with dependability and professionalism. Ability to effectively receive and carry out instructions from supervisors. Ability to embrace feedback and applies it to enhance work quality. Physical Requirements: Requires a high level of mobility in order to be able to provide outreach at various locations if needed. Preferred Qualifications: Experience engaging communities with high prevalence of violent crime in Central Arkansas (Pulaski, Jefferson, Garland, Faulkner, Saline, Grant, Perry, Lonoke counties). Experience working with community\-based organizations that provide mental and/or social services to individuals impacted by community violence. Participation in community violence prevention programs or research. Experience living or working in Little Rock – Central Arkansas Region. Experience working with diverse groups of people. Experience working as part of a multi\- disciplinary team, collaboratively practicing with health professionals, governmental, and/or community\-based organizations. At least two years of community/peer support or mentoring experience preferred. Experience using creative and interactive approaches to engaging and building rapport with clients/participants. Experience with case management, simultaneously managing a multitude of clients. Must be committed to serving a diverse group of patients, Must demonstrate a strong work history, including consistently reporting to work as scheduled and fulfilling assigned hours with dependability and professionalism. Responsibilities: Operate in a supportive role within an interdisciplinary research team; Provide peer support (status checks, encouragement/motivation, virtual and in\-person visits, outreach, and skill building activities) to current (in\-hospital) and recently discharged (post discharge) patients with violent assault injury; Conduct program eligibility screenings, informed consent, and completion of all required enrollment forms. Conduct brief bedside interventions that includes completing a needs assessments and reviewing a vetted resource guide; Track and review all participant encounters via data collection software’s such as RedCap, Excel, etc. Coordinate and execute all peer social skill building workshops for participants. Collaborate with Emergency Medicine, Trauma, and Psychiatry healthcare providers, hospital social workers, and community\-based organizations to recruit participants in the study and plan skill\-building workshops. May perform other duties as assigned. Additional Information: Salary Information: Salary is commensurate with education and experience.
