Assess, evaluate, diagnose, and develop a plan of care with goals; implement, supervise, and modify therapy plans based on patient response. Oversee and direct the work of Physical Therapy Assistants and other designated staff; collaborate with the health care team to ensure appropriate equipment and discharge planning. Document encounter findings, treatment plans, diagnoses, and prognoses in accordance with policy; communicate progress with referring providers and care conferences. Educate patients and families, including clear home exercise programs and instructions to promote independence and self\-care. Maintain clinical space, participate in quality improvement activities, and contribute to committee or administrative functions as assigned.
