Position Description

The Integrated Health Services Program Manager is a key programmatic leadership role aimed to support the day\-to\-day operations of a portfolio of health, social, and evidence\-based programs including case management fee for service, and programs supporting informal caregivers. The Program Manager also plays a critical role in supporting growth and future programs, particularly in the implementation of new integrated health services.

The goal is to develop a System of Care that is inclusive of a comprehensive network of community\-based services and supports organized to meet the needs of the population of focus such as social services, mental health, health care, and medical services that effectively builds on the individuals’ strengths to reinforce healthy aging in the community.

This individual will report to the Director of Integrated Health Services and work closely with the team and cross\-functional teams to ensure the successful implementation and execution of these critical projects.

  • Effectively manage time and showcase organizational skills by utilizing Project Management.
  • Proficient with operating and utilizing a Customer Relationship Management (CRM), Case Management Database/System(s), and/or Electronic Health Records (EHR).
  • Proficiency in MS365 team’s environment.
  • Experienced in managing programs for Medicare and/or Medi\-Cal eligible population.
  • Essential Functions and Responsibilities

    The duties below are representative of the range of responsibilities expected of the candidate but are not intended to be all\-inclusive. Key responsibilities include the ability to:

  • Supervise and manage day to day operations of Integrated Health Services programs encompassing care management, community and informal caregiver support at Sourcewise.
  • Develop and implement comprehensive program plans, timelines, and budgets for each initiative, ensuring alignment with organizational goals and stakeholder expectations.
  • Monitor and evaluate the performance of the Integrated Health Services Department program, using data\-driven approaches to identify areas for improvement and implement necessary changes.
  • Collaborate with cross\-functional teams, external partners, and stakeholders to coordinate resources, resolve issues, and drive project progress.
  • Monitor project performance, identify risks and issues, and develop mitigation strategies to ensure successful delivery.
  • Manage new client intake; oversee and streamline processes with referral sources.
  • Manage medical billing and data entry processes.
  • Monitor service delivery and costs incurred for each client and the program.
  • Track program enrollments and activities, ensuring billing data accuracy.
  • Continuously evaluate processes and implement improvements to enhance project efficiency and effectiveness.
  • Represent the organization in external meetings, presentations, and conferences related to special projects.
  • Monitors implementation of Care Plans; Recommends revisions or updates as necessary to accomplish the client’s goals.
  • Monitor caseloads to ensure client safety and program compliance.
  • Demonstrates capability to inspire staff to pursue excellence in goals and productivity.
  • Capable of coaching and improving staff in need of professional development.
  • Places high priority on overall team performance.
  • Train new team members of the department based on their role, as required.
  • Work in partnership with Case Manager(s) and Director to forecast caseload and make operational adjustments accordingly.
  • Participate in staff meetings, workshops, seminars, outreach events, and presentations.
  • Contribute as a facilitator of the Sourcewise Caregiver Support Group as needed.
  • Engage and lead Integrated Health Services team in program development and enhancement activities.
  • Act as liaison between ECM staff and Managed Health Plans to ensure compliance with program regulations.
  • Responsible for coordination, review, and accuracy of all ECM claims are entered timely for processing monthly.
  • Collaborate with Director and Community Development for proposals, applications, renewals, reporting, events, and opportunities that increase the visibility and sustainability of the program.
  • Recruit, manage, develop and evaluate staff for these programs including providing support, training and performance management to build the capacity of staff to accomplish program goals.
  • Ensures contract compliance and mitigates risks in collaboration with cross\-functional teams to achieve program Key Performance Indicators (KPIs) such as client growth.
  • Prepare weekly, monthly, quarterly, and annual YTD statistical performance reports and other necessary reports as requested.
  • Ensure accurate and timely maintenance of client records in accordance with mandated policy and procedures.
  • Support and participate in program audits and conduct Quality Assurance and Utilization Reviews of program services.
  • In partnership with the Director ensure program operations align with regulatory requirements including HIPAA, Medi\-Cal \& Medicare standards, and relevant agency policies.
  • The following duties are considered marginal (non\-essential) functions of this position. They may be reassigned as part of the reasonable accommodation process without altering the fundamental nature of the role: facilitating the Sourcewise Caregiver Support Group; collaborating with Community Development on proposals, applications, and grant renewals; and participating in outreach events, workshops, and external presentations. Other duties may be assigned consistent with the nature and level of this position. Marginal (non\-essential) functions may be reassigned as part of the reasonable accommodation process.

    Qualifications

  • Bachelor’s degree in Healthcare Management, Public Health, Human Services, Social Work, Business Administration, Operations Management, or a related field; or in lieu of a four\-year degree, six (6\) years’ experience working in a relatable field.
  • Minimum four (4\) years of progressively responsible experience in a supervisory or lead capacity in case management or a relatable field.
  • Minimum of four (4\) years’ experience working in social services or a relatable field.
  • Minimum of three (3\) years’ experience working with adults 18\+, seniors, and vulnerable populations.
  • Preferred experience working in case management and CalAIM.
  • Possess the case management skills necessary to develop and implement the case plan, consult as a member of a multi\-disciplinary team, and assess ongoing treatment planning efficacy.
  • Demonstrated a high level of professional responsibility and ability to oversee multiple programs, required.
  • Must have reliable transportation to travel to client and community sites within Santa Clara County. A valid California Driver's License may be required where driving is an essential function of the role. Driving records will be evaluated based on job\-related criteria consistent with business necessity and applicable law. Sourcewise will engage in the interactive process to determine whether reasonable accommodations enable the employee to perform essential job functions without undue hardship.

  • Ability to operate standard office equipment including computers, keyboards, phones, CRM and EHR systems on a regular basis.
  • Ability to move within an office environment and travel occasionally to community meetings, client sites, partner locations, and external presentations within Santa Clara County; driving may be required.
  • May occasionally lift or carry light materials (approximately 10 pounds), where applicable to job duties.

Work Environment

This position is primarily performed in an office environment with occasional attendance at community meetings, client sites, partner locations, outreach events, and external presentations throughout Santa Clara County. Employees may interact with older adults, adults with disabilities, government representatives, Managed Health Plans, and contracted service providers. Work schedule may be modified based on operational needs and reasonable accommodation requirements.

Pay: $80,253\.55 \- $96,649\.44 per year
