Position Overview
We are seeking an APP who is looking for something different than the traditional hospital or corporate healthcare environment.
This role offers a high degree of clinical autonomy, flexibility, and independence. As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and provide care in a way that prioritizes outcomes.

Care Coordination
- Collaborate with home health agencies, skilled nursing facilities, and referring providers
- Communicate treatment plans and updates to the broader care team
- Support transitions of care following hospital or SNF discharge
- Educate patients and caregivers on wound management and prevention
- Maintain accurate, timely, and compliant documentation
- Ensure documentation meets Medicare and payer requirements
- Follow HWMS clinical protocols and workflows
- Participate in periodic chart reviews and quality initiatives
- Active, unrestricted North Carolina license
- Board Certification (FNP, AGNP, PA\-C, or similar)
- DEA license (or ability to obtain)
- Valid driver’s license and reliable transportation
- Wound care experience or certification (CWOCN, CWON, WCC, or similar)
- Experience in home health, SNF, or post\-acute care settings
- Familiarity with Medicare documentation and billing practices
- Base salary: $110,000 – $140,000, based on experience
