  • Position Purpose:

    The primary purpose of this position is to provide clinical leadership to the households ensuring that the mission, vision, and values of the organization are supported through the clinical services of the household. The Care Team Manager shall participate in the planning, organizing, directing and facilitating the clinical operations of the household to ensure regulatory compliance, nurture a person\-centered and resident\-directed culture, strengthen the ability of the households to grow as a separate effective home, and promote individual and team learning within the households. All responsibilities will be performed according to local, state, and federal rules, regulations and guidelines that govern our state to ensure that the highest quality of resident care is delivered and maintained at all times. As the Care Team Manager of the Romney and Amstein Households, this position also supports the dementia care program for Marjorie P. Lee.

    Minimum Qualifications:

  • Must possess, at a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN diploma program and possess a current, unencumbered, active license to practice as an LPN in the State of Ohio. Registered Nurse licensure preferred.
  • Two years of experience as a supervisor and/or clinical manager in a hospital, skilled nursing care facility, or other related medical facility preferred.
  • Five years of experience in long term care preferred.
  • Certification in gerontological nursing is preferred, specialized training or experience in geriatric nursing with emphasis in dementia care.
  • Is knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long\-term care.
  • Knowledge and experience with person centered care principles preferred.
  • Is able to read, write, communicate effectively in the English language; is able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
  • Possesses a demonstrated ability to display leadership during a crisis and to handle crisis situations in a calm manner.
  • Possesses leadership and supervisory ability and is able to make independent decisions when circumstances warrant such actions.
  • Is able and willing to work independently with flexibility and integrity, as well as effectively and harmoniously with team members, residents, family members, support agencies, and government agencies/personnel.
  • Has patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents and is able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the Community.
  • Is able to move intermittently throughout the workday.
  • Is able to function independently and have flexibility, personal integrity, and the ability to work effectively with residents, team members, and support agencies.
  • Assists in the evacuation of residents during emergency situations.
  • Must be fully up\-to\-date with required vaccinations.
  • As a Nursing Care Team Manager you can expect to:

  • Provides knowledge and skills to team members in all households in specific areas of clinical accountability, including dementia care, infection control, safety, wound management, falls, MDS process or other areas as needed.
  • Guides the implementation of clinical policies and procedures to ensure safe, sanitary, and efficient practices supporting professional nursing care standards and assures that documentation, medication administration, resident nursing care, safety and sanitation, team member and talent development, MDS process, materials management, PIP and resident rights are in compliance with federal, state, local and ERS standards.
  • Ensures proper hiring, orientation, training, clinical competency, coaching, counseling, and performance appraisals are conducted in a timely manner and in a manner reflecting ERS’s and professional standards of practice.
  • Ensures that all clinical policies and procedures of the households are maintained. Interprets and communicates the clinical policies and procedures to all of the household as needed.
  • Provides ongoing monitoring of team member compliance for required trainings.
  • Works with the household clinical team members to ensure efficient and effective use of all resources to keep clinical staffing and supply budgets within planned expenditures.
  • Guides the clinical component of continuous quality improvement in the household in support of a systemic approach to quality clinical care.
  • Fosters collaboration, consensus, and negotiation amongst the care team and other departments as appropriate to address problem solving in the household.
  • Provides household supervision for nursing care. Provides direct nursing care as necessary.
  • Ensures the household teams are kept informed of the status of residents. Participates with other household members, the residents, and their family members in the care conference process to develop, implement, evaluate and update the residents’ comprehensive plan of care.
  • Assists in the admission, transfer, and discharge of residents.
  • Serves as the primary consistent clinical liaison between resident’s families and the clinical team. Communicates with families routinely and responds timely when contacted.
  • Helps lead the development, implementation, and evaluation of residents’ acute care plans. Reviews resident care plans for appropriate resident goals, problems, interventions, and revisions based on nursing needs in a timely manner. Assists in the evaluation of each resident’s physical and emotional status. Consults with the residents, family members, and physicians in planning the resident’s care, treatment, and rehabilitation as necessary.
  • Completes investigations and documentation for each fall and event requiring reporting timely and in a manner consistent with ERS and professional standards.
  • Reports and assists in the investigation of any abuse and/or misappropriation of resident property as evidenced by documentation. Reviews complaints and grievances made by a resident and makes a written/oral report indicating the actions taken to resolve the issues.
  • Identifies and evaluates patterns, trends, and clusters of infections and acts; accordingly, makes the appropriate plan of correction and works to prevent infections among residents, team members and visitors.
  • Collaborates with Infection Preventionist to produce monthly infection reports. Provides monthly and/or quarterly infection reports. Ensures that the appropriate external health agencies are notified when necessary.
  • Ensures that Community Infection Control policies/procedures are adhered to by all team members according to federal, state, and local agencies.
  • Collaborates with Wound Care Nurse Practitioner and interdisciplinary team concerning skin assessments and evaluations and assists in planning and developing the skin treatment to be performed for identified residents.
  • Examines resident(s) and records/charts and discriminates between normal and abnormal findings in order to recognize when to refer the resident to a physician for evaluation, supervision, or additional direction.
  • As Restorative Nurse, plans, develops, implements, and evaluates restorative care services. Coordinates programs with input from IDT and completes nursing assessment for restorative programs on residents whom members of the IDT have recommended for restorative and develops and implements a plan of care for each program specific to each resident’s individual needs.
  • Evaluates restorative programs and writes progress notes on a regular basis.
  • Models and ensures that others are providing medical treatment in a manner that honors resident preferences and individuality. Demonstrates a strong commitment to the highest clinical outcomes.
  • Provides household supervision for the dementia care program at Marjorie P. Lee.
  • Performs all administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.
  • Reviews and updates policies and procedures for all areas of responsibility annually.
