The Supervisor of Direct Care (SDC) is responsible for the development and coordination of all In\-home client services that support a client’s customized care plan. The SDC assures that the delivery of client care is conducted in a manner that meets Washington State, In\-home regulatory compliance and Wright's Home Care Agency's mission, standards, and policies. This position must be available during all hours of client care or appoints appropriate designee when not available.
This is a fast past job requiring a forwarding thinker, self\-starter, and a passion to provide the absolutely best support and life enrichment to our Senior clients. The candidate for this position must possess creative, problem\-solving capabilities. Having a CNA license is preferred as the successful SDC will use this knowledge, daily, in designing care plans and training Certified Nursing Assistants to insure our clients have the opportunity to live each day as independently as possible.
CLIENT CARE :
The Supervisor of Direct Care is responsible for the entire spectrum of In\-home Client Services. From the initial meeting to discuss how In\-home Care can improve the mental and physical well\-being of an aging adult, to conducting various assessments to determine a client’s functional limitations, daily customs and habits, and personal life history; the SDC designs a customized care plan including input from all individuals that will partner in caring for the client. This process is full spectrum because as our client’s needs change, their care plan must change with them. Care plan design includes: physical assistance, cognitive support, nutritional guidance, life enrichment activities, and various other safety and household support directives. One unique feature of Wright's Home Care Agency is the SDC with support of the Client Relations Specialist, finds ways to add positive experiences to our client’s lives through life enrichment programming that encompasses all facets of an individual’s being: physical, social, intellectual, artistic, and spiritual; including activities in and outside of the client’s home.
Our mission is to ensure the client, family, and care partners are satisfied with our services which is done through frequent client contact, supervisory visits, and scheduled care conferences.
The Supervisor of Direct Care reviews all incoming client related concerns, incidents, and change of condition, reported by caregiving staff and other interested parties. The SDC oversees and directs investigative action to be taken by their support team member, the Client Relation Specialist. With these findings, appropriate decisions can be made for updating the care plan, providing the CNA’s with additional training, and to ensure all client care partners and company management are kept up\-to\-date.
STAFF OVERSIGHT AND SUPERVISION:
One of the keys to Wright's Home Care Agency's success is through detailed training and oversight of our caregivers. Our training involves “hands\-on”, in the field training on each client’s care plan. Our caregivers must demonstrate the competencies and skills needed for client support. Whether the SDC is performing competency evaluations, orienting a caregiver to a new client care plan, or directing the Client Relations Specialist to schedule this training; the ultimate responsibility to ensure caregivers are competent and comfortable with their client duties, lies with the SDC. The SDC also collaborates with Human Resources in performing caregiver annual reviews, corrective retraining/counseling and quarterly in\-service training. The SDC provides an “open door” to employees, addressing all grievances/concerns in a timely manner and promoting a harassment free work environment. An important role of this position is identifying opportunities for our caregivers to experience skill/knowledge growth.
COMPANY GROWTH:
The SDC must maintain a strong knowledge of our community, its benefits, the services provided, and healthcare services overall. This position plays an important role in facilitating relationships in the community for improving the coordination of services to our clients. The SDC creates opportunities, when in the community and working with care partners, to generate appropriate referrals through contact with staff in hospitals, clinics, senior centers, and other appropriate individuals.
Through prompt and clear communication, the SDC identifies and reports to the Administrator when current systems are failing to support the company’s mission and objectives and offers suggestions for new programs that may improve services to clients and their families. This includes, providing reports, as requested, that evaluate our performance as a company and identify areas that need improvement through the company’s Quality Assurance Program.
The SDC develops relationships with compatible contractors and directs the completion of a qualification screening to evaluate if service partnership is appropriate. The SDC supervises and annually evaluates all direct care contractors, including the RN Delegator, to ensure coordination of services and compliance to regulatory standards and company policies.
OTHER RESPONSIBILITIES:
- Maintains current documentation in electronic record keeping system, including client notes, care plans, addendums, and client care instructional documents.
- Rotates into after\-hours call and must be available during all hours of operation. When not available, appoints a qualified designee.
- Supports office staff by answering phones and delivering messages, greeting and assisting all office visitors, performing inquiry intake, and documenting client/caregiver’s reports of concerns.
- Maintains professional development through self\-training and on\-going educational opportunities that meet company objectives and state regulatory requirements.
- Be involved with selling Wright's Home Care Agency to the community
- Must be available during all hours of operation; can appoint designee
- Genuine concern for and ability to work with aging and disabled adults
- Knowledge of the requirements for providing care and supervision to aging and disabled adults
- Ability to train, supervise, lead, and motivate people
- Ability to delegate responsibility while maintaining oversight of daily activities and major projects
- Ability to manage and prioritize a large, complex workload
- Knowledge of home care and other pertinent regulations
- Strong analytical and Internet/MS Office knowledge
- Strong customer service skills
- Team player, good communicator
- Meets Wright's Home Care Agency's employment criteria
- Certified Nursing Assistant, Social Worker, or Activity Director in a higher level care facility, such as, a Skilled Nursing Facility
- College degree a plus
