Job Summary

The Care Coordination program is a short\-term outreach program created to link Medicaid recipients residing in Baltimore City to health and wellness benefits. The MCH Referral/Data Assistant is a part of the administrative team that provides clerical support for the operation of the Care Coordination Program.

Duties and Responsibilities

Administration

  • Participate in professional development opportunities, build effectiveness in areas of client engagement and intervention, motivational interviewing, and trauma\-informed communication.
  • Participation in local/state meetings, conferences, and community meetings.
  • Participate in city\-wide initiatives/groups/committees as applicable.
  • Consistent and timely communication with supervisor
  • Care Coordination

  • Answer phones, greet and assist clients. Provide information and education about program services
  • Checks voicemail daily, return calls within 24 hours.
  • Provide administrative support to the HCAM outreach teams by documenting in the HCAM electronic medical record, utilizing the MMIS, CRM and E\-Medicaid systems
  • Knowledge of HCAM/CCP programs, Medicaid information and the Self\-Referral Process
  • Data Entry/Analysis

  • Run routine monthly reports, audit and perform quality steps to ensure programmatic documentation aligns with standard operating procedures
  • Enter outcome data into statewide database system; approx. 600\-700 clients per month
  • Coordinate the delivery of Service Response letters to referral Entities on behalf of the outreach team and Referral Coordinators; approx. 800 per month
  • Human Resources

  • Complete and submit timesheets timely and accurately.
  • Complete all the training courses required as assigned
  • Additional duties and responsibilities:

  • Additional duties as assigned by the needs of the program and leadership
  • QUALIFICATIONS:

    Education, Experience, Certifications and Licensures:

  • H.S. Diploma or GED
  • Experience working in an electronic medical record preferred
  • Word processing experience
  • Customer Service experience in a medical office setting
  • KNOWLEDGE, SKILLS \& ABILITIES

  • Demonstrated knowledge in the following areas: Community Outreach and Medicaid System Navigation.
  • Demonstrated knowledge and experience working with HealthChoice Medicaid recipients and Managed Care Organizations.
  • Organizational and Problem\-solving skills.
  • Strong knowledge of community resources.
  • Proven ability to work efficiently and meet deadlines.
  • Organizational skills with the ability to meet a demanding workload.
  • Excellent oral and written interpersonal and communication skills required.
  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365\).
  • Ability to work independently and as part of a team to achieve project success in outreaching clients identified for care coordination.

In compliance with Maryland's wage transparency requirements, the compensation for this role has been included in the posting.
