Job Summary and Responsibilities
The Virtually Integrated Care Coordinator role is crucial for assisting nurses in patient care coordination and supporting daily operational workflows within the innovative Virtually Integrated Care Program (VIC).
This position involves actively engaging with admitted patients to educate them on the VIC program and its benefits. The coordinator facilitates safe and timely patient discharge by coordinating follow\-up appointments, supporting medication coordination workflows, and facilitating communication among care teams, the pharmacy, and patients. Responsibilities
- Assist nurses with tasks such as verifying medication delivery and supports daily operational workflows within the Virtually Integrated Care Program
- Participates in team communication channels to stay aligned with priorities.
- Assist with administrative program coordination to support VIC leadership and nursing teams.
- Actively engages with patients admitted to educate on the VIC program and its benefits.
- Coordinate patient follow\-up appointments and support medication coordination workflows to facilitate safe and timely patient discharge.
- Utilize a virtual dashboard to facilitate patient care activities such as virtual rounding and safety checks.
- Facilitate communication between care teams, pharmacy, and patients to support discharge planning.
- Abstract meaningful data from the eHR to support VIC Program monitoring and reporting.
- Complete and support quality and safety audits as delegated by the VIC nursing team.
- Provide excellent customer service when interacting with patients, families, and healthcare team members.
- High School Diploma or GED equivalent
- Minimum three (3\) years experience as a medical assistant or other health related field
- Certified Nursing Assistant
- Certified Medical Assistant
- Clinical Physician office experience preferred
