Position Overview

The ID/DD Clinical Medication Coordinator is responsible and accountable for the coordination, supervision, and management of medication services for clients and their caregivers within the PA ODP Unskilled Medication Program in the home care setting. This position requires an ability to be flexible and work with all members of the health care team and provide ongoing supervision, education, support, and evaluation to the direct support staff. The ID/DD Clinical Medication Coordinator functions under the direct supervision of the Location and/or Area Clinical Director.

Essential Job Functions

Provide initial evaluation to determine appropriateness and safety of client and environment to participate within the medication administration program.

Assume responsibility in coordinating care to assigned individuals, working to support a goal\-directed support plan from admission to discharge which includes an ongoing assessment of individuals’ needs, focused on the medication administration program.

Perform on site supervisory visits to assess direct support staff performance and monitor client services.

Provide ongoing supervision, orientation, training, education, and evaluation of direct support staff, specifically pertaining to the administration and coordination of medications.

Ensure availability and proper operation of necessary equipment and supplies related to direct care.

Provide a written summary for individual clients and services based on needs, goals, and expected outcomes pertaining to the medication program in conjunction with the ISP to Supports Coordinators/Administrative Entities.

Evaluate the quality and effectiveness of services, analyzing appropriate data and information to identify opportunities for collaboration with all stakeholders in order to improve services and individual outcomes.

Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice.

Maintain a professional demeanor consistent with licensed nurse standards of practice.

Base decisions and actions on ethical principles and foster a non\-judgmental, non\-discriminatory climate in which care is delivered in a manner sensitive to socio\-cultural diversity.

Provide best practice in delivery of nursing care to the appropriate population and adhere to the standards of professional nursing practice.

Participate in after hours on\-call rotation.

Promote an environment of quality and safe client care through participation, development, and adherence to the QA plan and associated activities and metrics.

Ability to assess clients and provide direct client care as needed

Requirements

Diploma, Associate, or Bachelor’s degree in nursing from state accredited LPN/LVN or RN program.

LPN/LVN or RN licensure in the state of PA.

PA ODP Medication Program certificate or willingness to obtain within 90 days.

Valid Driver’s license and Acceptable MVR.

Current CPR/BLS certification.

Minimum of 1 year experience in nursing, preferably in home care/residential setting.

Preferences

Minimum of one year of experience working with ID/DD populations and/or the PA ODP programs is preferred

Physical Requirements

Must be able to speak, write, read and understand English.

Must be able to travel; company does not provide vehicles or transportation.

Frequent lifting, carrying, pushing and pulling of 25 pounds.

Occasional lifting, carrying, pushing and pulling of 50 pounds.

Prolonged walking, standing, bending, kneeling, reaching, twisting.

Must be able to sit and climb stairs.

Must have visual and hearing acuity.

Must have strong sense of smell and touch.

Must be able to sufficiently reposition patients and move equipment without assistance.

Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.

Occasional rapid movement and physical agility in response to the spontaneity of children and those with neurological impairment.

Environment

Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions

Possible exposure to blood, bodily fluids and infectious diseases
