Lead GPD (Grant Per Diem) Case Manager

The Salvation Army Veterans Housing is looking for a Lead Grant Per Diem Case Manager. This individual will lead, oversee, and coordinate the activities of veterans residing in the Transitional Housing Grant Per Diem programs. Act as a primary point of contact with the HCHV Grant Per Diem Program Liaison at the VA Hope Recovery Center, and Midtown Service and Treatment Center.

This is a full\-time position, 40 hours per week, with a starting salary of $53,000 per year. 2935 Locust Street, St. Louis, MO.

Essential Functions

  • Professionally complete case management functions such as intakes, entry/exit assessments, service/care plans, progress notes, case coordination notes, incident reports, notices, discharge summary, and post discharge arrangements within required timelines.
  • Guide case management team for all Veteran's programming and provide supports for productive day\-to\-day operations.
  • Govern an organized bookkeeping system for administration access while utilizing professional confidential practices.
  • Organize weekly class/group that incorporates life skills, employment skills, community resources for client, etc. and document as required.
  • Complete, record, and provide daily, weekly, monthly, quarterly, and annual activities, progress, outcome measurements and reports and required documentation.
  • Coordinate outreach, admission, and discharge with Health Care for Homeless Veteran (HCHV) Grant Per Diem (GPD) Liaison and staff, Midtown Service and Treatment Center staff.
  • Manage required census and client accountability according to operating protocol.
  • Oversee case management engagement as required and following up with case coordination to facilitate the process of clients meeting their goals.
  • Utilize community partnerships and resources to enhance support for clients.
  • Participate in weekly staff meetings (Transitional Housing program and treatment team), weekly/bi\-weekly/monthly supervision.
  • Provide updates, steps, insight of services to Program Directors (PD) regarding client progress, client off sight passes, and resources needed to accomplish client goals.
  • Maintain updated client files, utilizing professionally established practices for file maintenance.
  • Utilize recommended approaches and modules to support the goals of the GPD program.
  • Learn and maintain HMIS (Homeless Missourians Information System) data information and other programming software for operation.
  • Manage oversight of client daily living\-i.e. home living routine, cleaning of unit, saving plan, medication monitoring.
  • Review policies and procedures regularly and Support the mission of The Salvation Army and Veteran Residence program by treating all persons with respect and dignity, and without discrimination of any kind.
  • Perform other duties as assigned (e.g., in\-service training, fund\-raising/special events, disaster or seasonal activities.
  • Maintain confidentiality, proper client boundaries and comply with The Salvation Army Safe from Harm guidelines.
  • Uphold and support a positive work atmosphere by behaving and communicating professionally in a manner that fosters good relationships with co\-workers, clients, and supervisors.
  • Education and Experience

    Bachelor's degree in human service/social service field or related area plus one years of work experience is required. Experience working with recovering substance abusers and veterans is a plus. Combination of education and work experience will be considered. Certified Alcohol and Drug Counselor (CADC) or Recognized Associate Substance Abuse Counselor (RASAC) or LPC or LCSW is a plus. Requires some computer word processing, spreadsheet, and email experience. Excellent interpersonal and communication skills; understanding of the homeless and chemically dependent veteran population. Must be a team player, a self\-starter, able to handle several projects at the same time and perform well with minimum supervision. Must demonstrate ability to think independently and exercise sound judgment.
