Advance Care Pharmacy is a rapidly growing, dual accredited home infusion and specialty pharmacy located in Indiana, PA. We are looking for a home infusion/specialty intake specialist or coordinator. In this role, you will serve as the first point of contact for processing new patient referrals for home infusion and/or specialty pharmacy services. You will directly improve patient health outcomes by ensuring timely access to needed therapies, and by connecting patients with our pharmacists and other health care providers. This role ensures accurate intake of patient information, verifies insurance coverage, coordinates prior authorizations, including appeals, and facilitates smooth communication between prescribers, patients, and the pharmacy team. This is a full time position. It may be in person, remote, or hybrid.

- Receive and review new patient referrals from physicians, hospitals, or clinics.
- Accurately enter patient demographics, prescription details, and clinical information into the pharmacy systems.
- Verify insurance eligibility, benefits, and coverage for infusion medications and supplies.
- Initiate and track prior authorization and reauthorization requests with payers.
- Communicate with patients and caregivers regarding out\-of\-pocket expenses and available financial assistance.
- Communicate with prescribers’ offices to obtain missing documentation or clarify orders.
- Coordinate with pharmacists, nurses, and delivery teams to ensure timely initiation of therapy.
- Maintain compliance with HIPAA, accreditation, and all regulatory requirements.
- Provide excellent customer service to patients, caregivers, and healthcare providers.
- Ability to manage a high volume workload and remain organized.
- High school diploma or equivalent (associate’s degree or pharmacy technician certification preferred).
- Experience in home infusion, specialty pharmacy, or healthcare intake.
- Management experience preferred.
- Knowledge of medical terminology, insurance processes, and prior authorization procedures.
- Familiarity with authorization portals (CoverMyMeds, Availity, NaviNet, etc.)
- Strong organizational skills with attention to detail and the ability to multitask.
- Proficient in data entry and pharmacy/healthcare software systems.
