JOB DESCRIPTION: Care Navigator

Position: Care Navigator (Full\-Time)

Reports To: Lead Care Navigator

Location: Remote

Pay: $27\-$40/hr (depending on experience)

Role Summary

The Care Navigator is a critical member of the RTHM clinical operations team, serving as the primary point of coordination for patients navigating complex chronic illnesses. This role requires a highly organized, tech\-savvy professional who excels in a fast\-paced telehealth environment. You will be responsible for managing complex cases, streamlining patient workflows, and ensuring a seamless, high\-touch experience for our patients.

Essential Duties \& Responsibilities

* Complex Case Coordination: Manage end\-to\-end care coordination for patients with complex medical needs in a fully remote, telehealth setting.

* Patient Advocacy: Act as the primary point of contact for patients, assisting with scheduling, follow\-ups, and navigating their care plans via messaging, email, and video appointments.

* Clinical Support \& Documentation: \

  • Update and maintain accurate patient charts (medication lists, problem lists, social/family history).
  • Handle referrals, orders, lab lookups, and prior authorizations (PAs).
  • Manage pharmacy and third\-party lab communications efficiently.
  • * Communication Management: Monitor and respond to patient messages, voicemails, and emails within a 1\-business\-day SLA.

    * Process Adherence: Strictly follow and help refine established Standard Operating Procedures (SOPs) to ensure clinical safety and operational excellence.

    * Team Collaboration: Maintain active, real\-time communication with the remote team via Slack and participate in regular clinical and operational meetings.

    Required Qualifications \& Skills

    * Telehealth Expertise: Proven experience in care coordination specifically within a telehealth or virtual care environment.

    * Complex Case Management: Demonstrated ability to manage multi\-faceted patient cases with high attention to detail.

    * Remote Team Experience: Proven success working in a distributed, remote\-first environment.

    * Technical Proficiency: \* Tech\-savvy: Able to master new software and EMR systems quickly.

    * Slack Expert: Proficient in using Slack for professional, rapid\-fire team communication.

    * Professional Attributes:

    * Exceptional Communication: Strong written and verbal skills; able to convey complex information clearly.

    * Highly Organized: Ability to manage multiple priorities without losing track of details.

    Preferred Qualifications

    * Concierge Experience: Experience working with concierge or VIP patient populations.

    * Operational Leadership: Prior experience in a leadership role or clinical operations.

    * SOP Development: Experience drafting or improving Standard Operating Procedures to increase efficiency.

    * Medical Assistant Certification (CMA)

    Performance Standards

    * Responsiveness: High level of responsiveness to both patients and internal team members.

    * Quality \& Accuracy: Consistent accuracy in chart prep and administrative tasks.

    * Judgment: Sound independent decision\-making based on RTHM protocols.

    * Reliability: Consistent attendance and active participation in all scheduled remote meetings.

    Working Conditions

  • 100% Remote / Work\-from\-home.
  • Requires a dedicated, quiet workspace with high\-speed internet.
  • Daily use of computer systems, EMRs, and communication platforms (90%\+ of the day).
  • Employment Type: W2 role
