Job Summary:

We are seeking a compassionate, reliable, and skilled Licensed Practical Nurse (LPN) to provide high\-quality nursing care to patients in home and community settings in the EAST VALLEY. The Home Health LPN works under the supervision of a Registered Nurse (RN) and follows individualized care plans to promote patient health, recovery, safety, and independence.

This role is ideal for nurses who are comfortable working independently, value patient\-centered care, and enjoy the flexibility of home health nursing.

Key Responsibilities:

• Provide skilled nursing care in patients’ homes in accordance with physician orders and RN\-developed care plans
• Monitor, assess, and document patient conditions, including vital signs, symptoms, and responses to medications or treatments
• Administer medications and treatments as prescribed
• Educate patients and caregivers on disease processes, medications, and self\-care techniques
• Communicate effectively with patients, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in compliance with agency, state, and regulatory requirements
• Report changes in patient condition promptly to the supervising RN or physician
• Ensure patient safety and follow infection control standards at all times
• Travel to patient homes within the assigned service area

Qualifications:

• Active
LPN license in the state of Arizona

* BLS / CPR certification

• Minimum
1 year of nursing experience preferred (home health experience a plus)
• Valid
Arizona driver’s license and reliable transportation

* Arizona Fingerprint Clearance Card

• Current
TB test
• Must pass a
background check
• Authorized to work in the United States
• Experience with
Kinnser (preferred)
• Knowledge of
Electronic Visit Verification (EVV) is a plus
• Strong organizational, communication, and time\-management skills
• Ability to work independently with minimal supervision

Working Conditions:

• Frequent travel to patient homes
• Exposure to various home environments and potential infectious diseases
• Flexible scheduling; evening or weekend availability may be required
• Ability to manage patient behaviors professionally and safely
• Typical visit length:
1–2 hours per patient
