Job Summary:
We are seeking a compassionate, reliable, and skilled Licensed Practical Nurse (LPN) to provide high\-quality nursing care to patients in home and community settings in the EAST VALLEY. The Home Health LPN works under the supervision of a Registered Nurse (RN) and follows individualized care plans to promote patient health, recovery, safety, and independence.
This role is ideal for nurses who are comfortable working independently, value patient\-centered care, and enjoy the flexibility of home health nursing.
Key Responsibilities:
- Provide skilled nursing care in patients’ homes in accordance with physician orders and RN\-developed care plans
- Monitor, assess, and document patient conditions, including vital signs, symptoms, and responses to medications or treatments
- Administer medications and treatments as prescribed
- Educate patients and caregivers on disease processes, medications, and self\-care techniques
- Communicate effectively with patients, families, and the interdisciplinary care team
- Maintain accurate, timely documentation in compliance with agency, state, and regulatory requirements
- Report changes in patient condition promptly to the supervising RN or physician
- Ensure patient safety and follow infection control standards at all times
- Travel to patient homes within the assigned service area
- Active LPN license in the state of Arizona
- Minimum 1 year of nursing experience preferred (home health experience a plus)
- Valid Arizona driver’s license and reliable transportation
- Current TB test
- Must pass a background check
- Authorized to work in the United States
- Experience with Kinnser (preferred)
- Knowledge of Electronic Visit Verification (EVV) is a plus
- Strong organizational, communication, and time\-management skills
- Ability to work independently with minimal supervision
- Frequent travel to patient homes
- Exposure to various home environments and potential infectious diseases
- Flexible scheduling; evening or weekend availability may be required
- Ability to manage patient behaviors professionally and safely
- Typical visit length: 1–2 hours per patient
