POSITION SUMMARY:
Oversee the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) processes required by CMS. Ensures accurate resident assessments, timely submissions, regulatory compliance, and development of resident‑centered care plans. Assists in the planning, directing, and evaluation of programs related to resident care to provide consistency in nursing functioning in conjunction with the Assistant Director of Nursing (ADON).
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Review and develop individualized care plans addressing long‑term and short‑term resident needs.
- Communicate changes in care plans and assessments to clinical teams and families.
- Ensure documentation consistency and compliance with CMS, Colorado state regulations, and facility standards.
- Assist with mandated assessment forms and evaluations.
- Lead and facilitate interdisciplinary care plan meetings with nursing, therapy, social services, dietary, and other direct care staff.
- Review daily nursing reports and notes to identify changes requiring assessment updates.
- Monitor psychotropic use, hydration, nutrition risk, skin integrity, restraints, and wandering risk.
- Complete all required MDS assessments.
- Review IDCP (Interdisciplinary Care Plans) with residents and families during care conferences.
- Collaborate with ADON to achieve desired outcomes for residents, families, and nursing staff.
- Report QAPIs (Quality Assurance and Performance Improvement findings).
- Participate in monthly medication review.
- Coordinate and prepare discharge planning with physicians, mid‑level providers, social services, and home health agencies.
- Assist with the development, education, and scheduling of nursing staff.
- Assist with monthly staff training.
- Provide leadership, support, and communication across nursing departments.
- May work all shifts as Charge Nurse when needed, including weekends.
- Maintain regular attendance and punctuality.
- Supervisory responsibilities as needed.
- Ability to work independently, prioritize tasks, and meet strict deadlines.
- Current RN licensure with State of Colorado.
- Basic Life Support (BLS) certification.
- Minimum of two years of nursing experience in long‑term care or other healthcare settings.
- Supervisory experience preferred.
Performs other duties as assigned.

Ensures residents are treated with dignity, fairness, and respect. Adapts to changing workloads and prioritizes effectively. Knowledge of long\-term care operations and applicable state and federal regulations. Proficient in Microsoft Office; able to learn electronic health record (EHR) systems Reads and writes English proficiently. Demonstrates strong interpersonal skills and commitment to quality resident care.
