About Collaborative Medicine Collaborative Medicine LLC is an independent primary care and outpatient mental health practice located in Canton, Connecticut. We provide both in\-person and virtual care and focus on delivering compassionate, patient\-centered healthcare. Our philosophy is different from larger healthcare organizations. Our goal is to create a welcoming environment where patients feel heard, respected, and supported throughout their healthcare journey.
