We are seeking a dependable and detail\-oriented Medical Assistant to support our patient care coordination team. This role involves contacting enrolled patients, assisting with program participation, documenting patient communication, and coordinating updates with clinical staff.
Responsibilities • Contact enrolled patients by phone as part of the care coordination process. • Explain program steps in a clear, professional, and patient\-friendly manner. • Document patient participation information according to company procedures. • Verify patient information, including name, date of birth, phone number, and required account details. • Remind patients to complete assigned daily health monitoring tasks. • Follow up with patients who have missed required daily submissions. • Record patient communication notes accurately in the system. • Review submitted patient information for missing or incomplete entries. • Escalate patient questions or concerns to the appropriate clinical team member. • Coordinate with nurses, providers, care managers, and administrative staff as needed. • Maintain patient confidentiality and follow HIPAA guidelines at all times. • Provide courteous, professional, and compassionate support to patients.
Qualifications • Medical Assistant experience preferred. • CMA certification preferred. • Previous experience in patient support, care coordination, medical office, healthcare call center, or remote patient monitoring preferred. • Knowledge of basic medical terminology preferred. • Strong phone communication and customer service skills. • Good computer and documentation skills. • Ability to follow procedures and escalate concerns appropriately. • Dependable, organized, and detail\-oriented.
