A1 Service Coordination is a conflict\-free agency supporting individuals in the Traumatic Brain Injury (TBI) and Nursing Home Transition \& Diversion (NHTD) Medicaid Waiver programs. We serve participants across the Lower Hudson Valley and New York City.
We’re hiring a full\-time Assistant Service Coordinator (ASC) based in the Lower Hudson Valley to support our Service Coordination team in delivering high\-quality, person\-centered services.
This is an excellent opportunity for individuals currently in college or early in their careers who are looking to gain hands\-on experience working with individuals with disabilities, TBI, and older adults.
Important Travel Requirement
This role requires regular in\-person home visits across all seven counties: Westchester, Rockland, Putnam, Dutchess, Ulster, Sullivan, and Orange.
Candidates must be willing and able to travel to all counties as needed.
This is a hybrid position combining remote work with field visits. Mileage reimbursement is provided. Candidates should be comfortable driving up to about 1 hour each way from their home base.

- Communicate with participants, families, and providers as directed
- Support coordination of services and follow\-up on outstanding needs
- Learn waiver program requirements, compliance standards, and documentation practices
- High School Diploma required
- Associate’s Degree preferred (or currently pursuing a degree)
- Experience working with individuals with disabilities, TBI, older adults, or in human services preferred
- Strong organizational, communication, and interpersonal skills
- Willingness to learn and grow within the role
- Based in the Lower Hudson Valley with reliable transportation
- Comfortable traveling across all seven counties and entering homes
- Spanish or Russian language skills strongly encouraged
- Microsoft 365: Word, Excel (basic), Outlook, Teams
