Wellness Coordinators function in a non\-clinical capacity responsible for providing care coordination or administrative support to members with behavioral health and substance abuse needs.
Wellness Coordinators will also provide administrative support to the Behavioral Health Case Management department through assigning referrals, providing chart summaries and verification of member demographics.
Schedule: Monday through Friday a 40\-hour work week between the hours of 8:00 am to 5:00 pm CST
Location: work within a commutable distance to 1 Optum Circle, Eden Prairie, MN. (One to two days in the office per week.)

- as you take on some tough challenges.
- Make outreach calls and/or send letters to members and/or providers for care coordination.
- Carry a caseload of members, keep track of required tasks and timelines for each member, complete outreach and assessments according to timeline.
- Assist members by screening, answering questions, providing information and education about care coordination services, community resources, and provide benefit information in an accurate and courteous manner.
- Assist Care Coordinators with non\-clinical work, working and documenting in several applicable systems.
- Ability to screen or recognize risk and refer members with higher clinical needs to clinical team members.
- Perform provider and appointment searches.
- Complete administrative paperwork, preparing letters and other documents.
- Complete data entry into electronic records, including Minnesota Medicaid Information System.
- Provide supportive services to team members as needed.
- Attend/participate in required virtual team and Medica facilitated meetings and/or trainings.

- High School Diploma/GED
- 1\+ years of Call Center experience
- 1\+ years of computer proficiency experience to include working on multiple platforms and have experience with Microsoft Office (including Word, Excel, PowerPoint and Outlook)
- 1\+ years of experience\\knowledge of or ability to research and connect with community resources
- Ability to work 8am \- 5pm (Central Time Zone) Knowledge of or ability to research and connect with community resources
- Live within a commutable distance to 1 Optum Circle, Eden Prairie, MN. As you may be responsible for traveling into the office, one to two days per week
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Must live in a location that can receive a UnitedHealth Group approved high\-speed internet connection
- 1\+ years of experience in Medical/Behavioral health setting (i.e. hospital, managed care organization, or joint medical/behavioral outpatient practice)
- Experience working with individuals with disabilities or Severe and Persistent Mental Illness
- Able to handle sensitive issues with members and providers in a confidential manner
- Managed care experience
- Must demonstrate flexibility, ability to follow through on all tasks in a timely fashion, good attention to detail and a willingness to learn
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
- Demonstrated effective organizational skills and scheduling skills as observed in day\-to\-day work
