- Respond to referrals and service requests from older adults and individuals with disabilities via phone, email, and in\-person interactions.
- Assist clients with applications for public assistance and cost\-saving programs (e.g., Medicaid, Medicare Savings Programs, SSI, HEAP, SNAP), with a primary focus on improving economic security.
- Conduct home visits as needed to assess client needs and provide support.
- Maintain and manage multiple databases, ensuring accurate and up\-to\-date records.
- Collaborate with Aging staff, City of Cleveland departments, and community\-based organizations to coordinate client services.
- Maintain current knowledge of social services, as well as public, private, and governmental resources serving older adults and individuals with disabilities.
- Supervise interns and AARP trainees.
- Actively participate in initiatives aligned with the Age\-Friendly Cleveland Plan.
- Attend and assist in facilitating community presentations and outreach events.
- Complete monthly reports and audits to meet billable funding and performance targets.
- Develop and maintain partnerships with external organizations to enhance services for Cleveland’s older adults and residents with disabilities.

Examples of Duties
Under administrative direction, plans and administers a specific project or program for the Department of Aging. Serves as the administrative supervisor on projects or programs. Plans, assigns, supervises, and reviews activities of subordinate personnel. Monitors the project/program's operating budget and assists in the annual preparation of same. Designs, implements, and monitors procedures utilized in program evaluation. Develops and secures commitments from other City departments and agencies to provide services to program participants. Provides technical assistance or reviews program policies and procedures. Maintains current information and oversees the preparation of project status reports. Prepares financial and narrative reports and program applications. Communicates project/program status and information to municipal officials and/or public and private entities. May supervise employees assigned to the respective project. Performs other related duties as required.
Minimum Qualifications
Bachelor's Degree required, those in Social Work, Social Services, Gerontology, Counseling, Human Services, Psychology Public Health, Health Services, or closely related field from a four\-year accredited college or university are preferred; Master's Degree is preferred. Two years of full\-time progressively responsible experience in government or private sector program administration required. Three years of full\-time experience in human services is also required, working with the elderly and/or disabled adults is also preferred. (Substitution: Post graduate education in one of the above fields may substitute for work in government or the private sector and/or experience working with human services on a year\-for\-year basis) Must be able to operate a personal computer with Microsoft Office Suites. Valid State of Ohio Driver's License required. Must be able to lift and carry 30 pounds. Must comply with the fingerprint\-based records check under RCS 173\.394\. Strong verbal, written, oral, electronic, communication, and public speaking, planning, and organization skills are required.
