Key Responsibilities

• Communication Assessment: Conduct comprehensive evaluations of patients' speech, language, voice, fluency, and cognitive\-communication abilities in their home environment
• Swallowing Evaluation: Perform clinical swallowing assessments and develop safe eating and drinking recommendations to prevent aspiration
• Treatment Planning: Develop individualized treatment plans with measurable goals focused on improving communication abilities, swallowing safety, and cognitive function
• Therapeutic Interventions: Implement evidence\-based speech therapy techniques including articulation therapy, language intervention, voice therapy, dysphagia management, and cognitive rehabilitation
• Patient \& Caregiver Education: Provide expert guidance on communication strategies, safe swallowing techniques, diet modifications, and home practice activities
• Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
• Care Coordination: Collaborate effectively with physicians, nurses, dietitians, and healthcare team members to ensure comprehensive patient care
• Progress Monitoring: Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements

Required Qualifications

Licensure \& Certifications

• Graduate of an accredited Speech\-Language Pathology program (Master's degree required)
• Valid Illinois Speech\-Language Pathologist license
• ASHA Certificate of Clinical Competence (CCC\-SLP) or Clinical Fellowship eligibility
• Current CPR certification
• Valid driver's license and auto insurance
