The Clinical Manager Assistant performs multiple tasks in the office. The main role is to provide support for the Clinical Manager as well as performing other functions in the branch. The Clinical Manager Assistant will be the first line of contact for physician and patient calls, function as the inpatient coordinator, follow up on labs, durable medical equipment, and intake. Other duties include case conference prep, discharge/recert process, entering faxed orders, \& quality/performance improvement data entry. This non\-exempt position will report to the branch director.Clinical Manager Assistant Essential Functions:
Clinical
- Receive phone calls from physicians, facilities, staff, and patients/caregivers with appropriate follow up.
- Entry of verbal orders into Home Care Home Base.
- Ensure all referrals have the essential documents required. Follow up to obtain missing information to ensure a complete referral.
- Monitors status of patients in the hospital to ensure adequate care coordination. Including follow\-up to ensure transfer paperwork is completed and sent to the facility.
- Monitors status of pending labs facilitates needed follow\-up and communicates result to physicians and other personnel, as appropriate.
- Assist the Clinical Manager in the Discharge/Recert Audit process.
- Complete assigned workflow timely.
- Assist the Clinical Manager with managing various system reports for the effective care coordination and utilization management that promotes positive patient outcomes.
- Provides direct patient care, as necessary, in accordance to scope of practice, plan of care and physicians order, and in coordination with other members of the health care team.
- All other duties as assigned.
- Prepares patient list and necessary document for weekly case conference.
- Monitors incoming faxes, reports information to appropriate personnel and enter faxed orders into Homecare Homebase.
- Assist in quality/performance improvement process by entering data as needed.
- Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which he / she practices
- Have at least 1 year of home health experience.
- Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification required.
- Ability to organize tasks, develop action plans, set priorities and function under stressful situations.
- Efficient in use of Microsoft products (i.e. Word, Excel, etc.)
- Ability to work independently, under general instructions, and with a team.
- Computer proficiency: comfortable working with and on computers as well as within multiple systems simultaneously
- Performs both field and remote work as needed
- Must read, write, and speak fluent English.
- Must understand the issues related to the delivery of home health and hospice services and be able to solve problems effectively.
- Must possess knowledge of Medicare guidelines governing home health agencies.
- Must have exceptional customer service skills and be able to communicate well with referral sources, patients, family members, and agency employees.
- Must be organized, detail\-oriented, and possess effective communication skills both orally and in writing.
- The ability to communicate with a diverse number of individuals is required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
