McGregor PACE is a community\-based program that provides comprehensive, in\-home healthcare services to older adults as an alternative to nursing home placement—empowering seniors to remain safely and independently in their homes.
Position Summary:
We are seeking a highly motivated and dedicated Center Assistant / Homecare Staffing Scheduler to join our team. This is a dual\-role position that combines direct participant care in our Adult Day Center with coordination and administrative support for home care services. This role is ideal for someone who enjoys both hands\-on care and behind\-the\-scenes coordination.
Key Responsibilities:
Center Assistant Duties:
- Provide personal care, assistance with ADLs, and ensure participant safety in the Adult Day Center
- Greet and assist participants upon arrival, creating a welcoming and supportive environment
- Support implementation of care plans and healthcare protocols
- Provide front desk/reception coverage as needed
- Coordinate and manage Home Health Aide (HHA) schedules and services
- Serve as a liaison between participants, home care agencies, and internal teams
- Communicate scheduling updates and address service\-related concerns
- Reconcile services provided with services billed; submit accurate billing information and resolve discrepancies
- Maintain and organize home care documentation, records, and data tracking
- Collaborate with transportation and care teams to ensure participant needs are met
- Assist with data collection, reporting, and outcome tracking
- Attend IDT meetings as needed to communicate participant care needs
- Process service requests and changes in the absence of the Home Care Staffing Supervisor
- Provide in\-home support as a Home Health Aide on an as\-needed basis
- Manage appointment scheduling and maintain accurate, confidential participant records
- Perform data entry, filing, and general office support
- Maintain effective communication between staff, participants, and external partners
- High School Diploma or equivalent (required)
- HHA, CNA, or STNA certification (preferred)
- Minimum of one (1\) year experience working with a frail or elderly population (required)
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal skills
- Basic computer skills, including database and word processing proficiency
- Ability to build collaborative relationships with internal teams and external partners
