About the Role
Legara, Inc. is seeking a California\-licensed Psychiatric Nurse Practitioner (PMHNP) to provide outpatient psychiatric care for an underserved FQHC patient panel. This role offers flexible scheduling, robust administrative support, and the opportunity to make a meaningful impact on patients who face significant barriers to mental health care.
You’ll see a diverse population of predominantly low\-income, Spanish\-speaking adults, children, and teenagers with moderate to severe mental health needs, including substance use disorders and mood disorders.
Key Details
* Schedule: Weekdays; variable with multiple week notice, mutually agreed upon, 0\-3 days per week
* Hours: No nights, weekends, or after\-hours inbox work
* Setting: Fully remote (must be located in California)
* Engagement Length: Open\-ended; flexible to scale up or down
* Credentialing: 4 \- 8 weeks before training begins
* Compensation: Encounter\-based pay averaging $130 \- $195 per clinical hour, $117 per non\-clinical hour spent completing trainings, weekly payments, Form 1099\-K issued annually
* Support: Administrative and scheduling team manages patient bookings, templates, reminders, and paperwork
What You’ll Do
- Provide Courtesy Medication\-Management Follow\-Ups: 15–30 minutes each, approx. 13 per clinic day
- Chart directly in the health center’s EMR (NextGen)
- Join brief care\-team huddles and collaborate with primary care, behavioral health, and case management staff
- Provide culturally responsive care; Spanish fluency is a plus, but not required
- Follow the FQHC’s standard medical staff oversight and QA policies
- Current, unrestricted PMHNP license in California
- Active DEA registration
- Strong technological skills and ability to quickly learn complex healthcare software (NextGen experience preferred)
- Comfortable serving Medi\-Cal and uninsured patients
- Secure broadband internet, webcam, and a private workspace for telepsychiatry
- Malpractice coverage: $1M/$3M base policy
- Professional fluency in Spanish (speaking, reading, documentation)
- Previous experience working in safety\-net healthcare or with underserved populations

- You will be compensated through a form 1099 and not classified as an employee. Will that work for you?
- Perform an internet speed test from the internet connection where you would hold telehealth visits. What are the download and upload results?
- What would be the ideal schedule for you in terms of specific weekdays, time blocks on those days, and approximate total number of hours each week?
- • We're looking for a PMHNP interested in substituting as needed with multiple week notice for current team members, with an interest in building your own case load and more consistent schedule a month or two down the line, should the health clinic feel like a good fit for you, and vice versa. What interests you about this arrangement?
- As outlined in the job description, you will be compensated solely with a per\-encounter rate. We reasonably estimate that once you get going, if you were to work a full day, you would see 17 patient per day. At $65/enc, that would be $1,105 per day.
