General Summary of Position: The Pediatric Nurse (LPN/RN) works within a Care Team to provide Nurse Visits, Triage, Case Management, patient education, and patient care support to the Care Team within the scope of nursing.
Major Areas of Responsibility:
* Floor Nursing Duties: Administering immunizations, Pediatric screenings: vision, hearing, and a few behavioral health assessments, Health Education, Medication administration, and Nurse visits. Serving as liaison between providers and their patients. Executing nursing care under direction of the provider such as: medication injections, wound care and dressing changes, IV infusions and assisting in procedures.
* Health Education: Educates patients regarding health promotion, illness prevention, medical diagnoses, medication administration, treatment procedures, disease management, and community resource utilization. Utilizes provider\-based protocols and/or direct provider order. Communicates abnormal lab results and treatment plan to patients as outlined by the provider and/or protocols.
* Case Management: Coordinates and manages ongoing patient care to patients outside of the Office Visit utilizing community resources, patient education, and Nurse Visits.
* Medication Administration:Assists in medication and immunization administration (where licensure is required) and Works alongside medical assistants on requests for medication refills per standard work.
* Triage:Manages the clinical phone line. Assists pharmacies, responds to patient requests, conducts triage and conveys messages to and from providers, clinics, specialists, and other care team members. Responds to telephone calls and patient walk\-ins: Assesses patient, documents findings in EMR, reports findings to physician, carries out standing orders and other physician orders.
* Nurse Visits:Schedules and performs Nurse Visits where licensure is required. Assesses patients, documents findings in EMR, reports findings to physicians, carries out standing orders and other physician orders. Provides nursing care to patients who present through the Nurse Visit Schedule. This may include, among other types of visits: BP checks, wound care, patient education, medication administration, triage, pregnancy tests/counseling, Nutrition counseling, Urinalysis and conveying test results. Collaborates with medical support staff and assists with prioritizing clinical tasks to ensure efficient work flow. Documents all activity appropriately in the Electronic Medical Record. Promotes clinical excellence.
* Staff Development: Assists in education of Care Team members. Participates in care team meetings, monthly nurse meetings, clinic\-wide quarterly meetings and other meetings as directed by site manager.
* Member of Care Team:Works effectively as a Care Team member. Assists MAs with duties during high volume or high acuity to minimize patient wait Uses nursing judgment to delegate responsibilities to MAs and Facilitator/Patient Advocate, as needed to aid patient schedule flow.
* Maintains Required Qualifications: Takes initiative to obtain/maintain required qualifications to perform duties (RN/LPN licensure, CPR certification, TST certification).
* Participates in Quality Improvement: Takes initiative and takes on responsibilities to improve clinic functions and procedures with evidenced based practice when directed.
* Other Duties as Assigned: Vaccine Program management and medication Delegates responsibilities when applicable.
* Ability to assume leadership responsibility: possible opportunity for leadership (e. Site manager, Vaccine coordinator, Chronic Care Management, Diabetes education etc.)

- A nursing diploma, bachelor’s degree or associate’s degree in nursing, or the equivalent is required.
- A current, valid LPN or RN license in the state of Georgia is required.
- BLS certification is required
- 2\-3 years of previous inpatient nursing experience is appreciated.
- Community Health or Public Health experience and GRITS training are desirable.
- Additional board certifications are favorable.
- Bilingual(English and Spanish) abilities preferred
