Job Title: Certified Medical Assistant – Remote Patient Monitoring (RPM) / Chronic Care Management (CCM) \- FULL TIME ONLY

Compensation: $20\+ / hr; incentivized earnings

Position Summary:
We are seeking a Certified Medical Assistant (CMA) who is passionate about chronic care, patient engagement, and making a real difference in patients' lives. This is a hybrid role where you’ll spend one day per week in the field conducting in\-person onboarding and device setup, and the remainder of the week performing telephonic RPM and follow\-up tasks from our office. We are willing to build this role around your home location.
Key Responsibilities:
- Conduct patient onboarding and education for RPM devices (blood pressure cuffs, glucose monitors, etc.) in the field (1 day/week)
- Review patient vitals data via RPM software and assist in triaging abnormalities
- Perform routine telephonic outreach, monthly check\-ins, and care coordination under NP supervision
- Document encounters and clinical notes in the EMR with accuracy and timeliness
- Communicate updates with the supervising provider and escalate care as needed
- Collaborate with billing and administrative teams to ensure compliance and tracking
- Provide basic tech support for patients unfamiliar with RPM devices or portals
- Maintain HIPAA compliance and high standards of patient confidentiality
- Certified Medical Assistant (CMA) credential required
- 1\+ year of clinical experience (RPM or CCM experience a plus)
- Excellent communication skills—especially for phone\-based encounters
- Comfortable using EMRs and digital RPM platforms
- Valid driver’s license and reliable transportation for field days
- Strong organizational skills and attention to detail
- Full\-time, Monday–Friday
- 1 day per week in the field (home visits for onboarding/training)
- Remaining days in\-office performing phone\-based encounters and documentation
- Supervision and collaboration with Nurse Practitioner\-led clinical team
