Salary range: The salary range for this position is $140,000 to $165,000\. This position includes membership in the Health Sciences Compensation Plan, which provides for eligibility for additional compensation. The salary range listed is for base compensation only and may be adjusted depending on qualifications and experience. The salary range does not include variable compensation and is subject to the individual department compensation plan.

POSITION DESCRIPTION

The Betty Irene Moore School of Nursing at UC Davis seeks an innovative and highly motivated master’s or doctorally prepared Family or Adult Gerontological Nurse Practitioner to join the faculty. This Nurse Practitioner (AGPCNP) will serve in a unique role providing clinical care to older adults or families, clinical teaching activities, and clinical research. While all Nurse Practitioners are welcome and encouraged to apply, the school is particularly interested in FNPs or AGPCNPs who have an interest in healthy aging, dementia care, and chronic disease management in older adults or unhoused people. They will have recognized expertise in the care of older adults, particularly the holistic assessment and management of older adults with dementia, chronic co\-morbid diseases, and/or frailty. This position will split their time between clinical responsibilities, teaching, and research.

This position will be responsible for providing patient care in the healthy aging clinic collaborating with geriatricians, advanced practice providers, and other clinicians and staff. In the clinical practice environment they will work under the direction of the Healthy Aging Clinic leadership in a team\-based care environment, precept students in the clinic, serve as a clinical resource in providing clinical care for older adults including co\-developing multifaceted treatment plans, providing patient education, and participate in clinical care huddles; they will report to the Associate Dean for Practice for practice related issues. There may also be open positions in the or in the nurse run mobile clinic. This position will also have teaching responsibilities as assigned in the SON programs (DNP\-FNP, PhD, MEPN, and PA programs), reporting to the Associate Dean for Academics. They will also be responsible for developing a program of research in collaboration with the Healthy Aging Clinic or mobile clinic leadership or other researchers with similar research interests.

Part\-time and full\-time, non\-tenured positions at the Assistant and Associate rank in the Health Sciences Clinical Professor (HSCP) Series or full\-time Clinical X Series are available at UC Davis Health, which is located on a modern, 140\-acre campus in Sacramento, California. The Sacramento campus is home to the School of Nursing, School of Medicine, UC Davis Medical Center (including a level\-I trauma center and burn unit, and a full range of outpatient services), a fully accredited Children’s Hospital, and an NCI\-designated Comprehensive Cancer Center.

The budgeted salary range for this position is $140,000 to $165,000\. Assistant or Associate salaries are commensurate with experience and qualifications. This position includes membership in the Health Sciences Compensation Plan.

CLINICAL RESPONSIBLITIES:

Successful candidates will provide clinical care to geriatric patients, caregivers, and precept students. Duties will include:

  • Provides independent advanced level nurse practitioner clinical care to older adult patients.
  • Works collaboratively within the team.
  • Functions as an advanced practice nursing leader in collaboration with medical director and other leaders.
  • Provide direct care to patients as indicated in alignment with the team\-based care collaborative approach.
  • Collaborates with leadership and clinic staff to develop systems and methods to assist/improve resource utilization and additional services.
  • Collaborates with leadership to encourage practice environments that support shared decision\-making, employee engagement, and interdisciplinary coordination of care.
  • Precept advanced practice students
  • Documents care comprehensively in the electronic health record.
  • Specific to Healthy Aging Clinic – Participate as a member of the team in clinic wide initiatives:
  • Dementia care
  • Behavioral Health Integration

  • Facilitate caregiver’s capabilities to provide safe care to their older adult relative or friend working with the AGCNS and other providers.
  • Advance Care Planning
  • Ensure that patients and/or caregivers understand medication management, wound care, incontinence care, behavior management and other identified patient challenges as indicated; may refer to other clinic providers.
  • Care coordination to help caregivers navigate the health system.
  • As needed, serve as a clinical expert resource for staff and students and provide geriatric screening examinations to older adults. These exams may include examples such as but not limited to: o Mini Mental Status Exam or MOCA or Mini\-Cog o Unintentional Weight Loss Screening Tool o Functional Screening (Lawton IADL, GAF) o Geriatric Depression Scale o Nutrition assessments o Pain assessments
  • RESEARCH AND SCHOLARSHIP RESPONSIBILITIES:

    For Healthy Aging Clinic \- Develops a program of clinical research:

  • In collaboration with the Family Caregiving Institute, Associated Medical Departments, SON Associate Deans for Research and Practice, develops a program of funded research focusing on older adults and related topics of interest in the care of older adults OR
  • In collaboration with other members of the healthy aging community and SON, develops a program of funded research.
  • Participates in Family Caregiving Institute or other UCDH or SON research consortium activities.
  • Disseminates research findings to national audiences.
  • For Mobile or other Clinic \- Develops a program of clinical research appropriate to clinic patients or operations.
  • TEACHING, MENTORING \& SERIVCE RESPONSIBILITES:

  • Teach and mentor students in areas of expertise as assigned by the Associate Dean for Academics.
  • Provide service to the School of Nursing, Health System, and University through committee participation as assigned.
  • Provide service to the community and profession.

    Basic qualifications (required at time of application)

    Graduation from an accredited school of nursing with a BSN or AND master's degree is required, doctoral degree (e.g. PhD, DNP, EdD) is preferred. The candidate must meet the following qualifications:

  • Doctoral degree in nursing or related field preferred with AGPCNP or FNP certification and/or equivalent combination of experience / training.
  • Experience in geriatric nursing care required.
