Home Health Nurse Registry in Sunrise, FL seeking a Bi\-lingual Patient Care Coordinator.

Applicants must meet the following basic requirements:

  • High School Diploma
  • 2 Years experience in Customer Service / Reception
  • AHCA Level II background screening done
  • Eligible to work in the United States
  • Microsoft Office Competent
  • Administrative skills such as filing, data entry and scheduling
  • MUST be bi\-lingual (english/spanish)
  • The Assistant Care Coordinator responsibilities may include:

    The Patient Care Coordinator will be responsible for referring qualified and compatible caregivers to clients in home.

  • Initiating and maintaining communication with client, family members, advocates and case managers
  • Maintaining active client and caregiver list
  • Creating schedules on scheduling platform (HHAeXchange)
  • Requesting service authorization for clients
  • Preparing new client folders
  • Sending notifications to client doctors and insurances
  • Completing new client intake
  • Maintaining employee roster for active caregivers
  • Knowledge, Skills and Abilities:

  • Basic/Intermediate computer skills with a willingness to learn our intake and patient care systems
  • Strong verbal/written communication and listening skills; including excellent impersonal skills and telephone communication
  • Excellent organizational and time management skills
  • Ability to identify and resolve problems
  • Ability to effectively organize and prioritize tasks in order to complete assignments within the time allotted and maintain standard workflow
  • Ability to establish and maintain effective working relationships with patients, case managers, and coworkers
  • Ability to maintain confidentiality of sensitive and protected patient information
  • Ability to work effectively as a team player and provide superior customer service to all staff and leadership
  • Spanish speaker is MUST!
  • Job Type: Full\-time

    Pay: $46,800 Annually

    Schedule: Monday \- Friday, 9am \- 5pm

    Work Location: In person
