National Hospice and Palliative Care is one of Northern California’s leading providers of hospice services. At this time, our rapidly growing company is in immediate need of professional and self\-motivated Hospice Registered Nurse.
Immediate needs in the following areas: San Jose, Santa Clara, Sunnyvale, Mountain View, Palo Alto, Redwood City, Milpitas, Fremont, Hayward, and surrounding areas.
Responsibilities include:
- Performing initial in\-home evaluations and establishing an individualized care plan for each patient by collaborating with caregivers to provide the best possible patient care.
- Communicating daily with the interdisciplinary team including physicians, social workers, chaplains, nursing assistants, volunteer coordinators, and other hospice nurses
- Visit patients and evaluate their condition and individual patient needs

- Minimum one year of experience as a Registered Nurse
- 1 year of Hospice experience preferred

On\-going training and clinical support make our Case Managers able to work independently while providing care to a wide range of complex cases.
