Provide skilled physical therapy services to patients in their homes to improve mobility, reduce pain, and prevent disability. Educate patients and their families on treatment plans and home exercise programs. Collaborate with a multidisciplinary team of nurses, aides, and other healthcare professionals. Maintain accurate and timely clinical documentation using our state\-of\-the\-art EMR system. We're Looking For Someone With: For Physical Therapists (PT): Current CA State license in good standing as a Physical Therapist. Doctorate or Master's degree in Physical Therapy from an accredited program. Strong clinical skills and a passion for patient\-centered care. For Physical Therapy Assistants (PTA): Current CA State license/certification in good standing as a Physical Therapy Assistant. Associate's degree from an accredited PTA program. A team\-player mindset and excellent communication skills.
