Overview

HouseCall MD is seeking a Geriatric Physician MD or DO to join our clinical care team in San Jose CA and surrounding areas.

We are a physician\-led mobile medical group providing primary care services to elderly patients in private homes assisted living facilities and residential care settings. This role is designed to maximize physician autonomy while minimizing administrative burden.

Key Responsibilities

• Perform preventive routine and urgent care visits for geriatric patients
• Manage chronic conditions and develop proactive care plans
• Assess safety of patient living environments
• Educate patients families and caregivers
• Coordinate care with interdisciplinary teams across settings
• Document visits in the EHR system

Qualifications

Required

• MD or DO degree from an accredited medical school
• Active state medical license
• Active DEA license

Preferred

• Board Certified or Board Eligible in Internal Medicine Family Medicine or Emergency Medicine
• Experience in geriatrics primary care or mobile care

Work Model

• Provide care in homes assisted living and residential care settings
• No hospital privileges required
• Not a hospitalist role focused on primary care
• Full control of scheduling and daily workflow
